新疆地区三级甲等综合医院ICU质量控制指标的比较

Comparison of ICU Quality Control Indicators among Third-class General Hospitals in Xinjiang Uygur Autonomous Region

目的比较新疆乌鲁木齐市3所三级甲等综合医院ICU病房的质量控制指标的实际临床应用效果的差异,分析出现差异的可能原因,探讨提高指标应用效果的应对措施。方法选择新疆乌鲁木齐市3所三级甲等综合性教学医院,分别记为A、B、C,研究对象为2016年1-3月3所医院收治患者的相关资料,由3所医院的信息科统计本院的收治总患者数量、总床日数、ICU总床数等信息,ICU病房护士负责收集患者的诊疗等相关信息,对采集来的信息进行整理、统计分析。结果 C医院的急性生理与慢性健康(acute physiology and chronic health evaluation,APACHE)Ⅱ评分≥15分患者收治率较低,非计划气管插管拔管率较A、B医院高;A医院各置管使用率和感染率等指标较B、C两个医院质量控制效果好。结论目前新疆地区三级甲等综合医院的ICU质量控制有待加强,需要增加医生和护士的人员配置、改善诊疗环境。

Objective To compare the actual clinical application effect of ICU wards in three third-class general hospitals in Urumqi,to analyze the reasons for the possible differences,in order to improve the application effect of indicators.Methods Three third-class general hospitals were selected and marked as A,B,C.The clinical data of patients,total number of bed days and ICU beds were collected from January to March 2016.ICU nurses were responsible for collecting patient data and other related information.The information was organized and analyzed.Results The score of acute physiology and chronic health evaluation(APACHE)Ⅱwas greater than 15 points in hospital C,which was at a low level.The unplanned endotracheal extubation rate in Hospital C was higher than that in the A and B hospitals.The usage rate and infection rate of catheters in hospital A were better than those in B and C hospitals.Conclusions The quality control of ICU in third-class general hospitals needs to be strengthened.It is necessary to increase the staffing of doctors and nurses and improve the diagnosis and treatment environment.
