计算机远程认知训练在老年2型糖尿病合并轻度认知功能障碍患者中的应用效果研究 被引量:6

摘要 目的:探究计算机远程认知训练在老年2型糖尿病(T2DM)合并轻度认知功能障碍患者中的应用效果。方法:选取2016年1月-2017年2月在笔者所在医院接受治疗的老年2型糖尿病合并轻度认知功能患者80例,采取随机数表法将患者随机分成对照组和试验组。对照组实行集体教育方式,试验组患者通过计算机远程认知训练实施个体化综合管理干预,观察比较两组患者干预前后的认知功能、糖尿病自我管理能力、血糖控制情况(糖化血红蛋白)及生存质量等数据。结果:两组患者干预前的Mo CA、MMSE得分比较差异无统计学意义(P>0.05);干预后试验组患者的Mo CA、MMSE得分均高于对照组(P<0.05)。干预后试验组患者的糖尿病自我管理能力得分高于对照组(P<0.05)。干预后试验组患者的糖化血红蛋白值低于对照组(P<0.05)。干预前,两组患者的生存质量评分比较差异无统计学意义(P>0.05);干预3个月后,试验组患者的生存质量评分高于对照组(P<0.05)。结论:计算机远程认知训练能够增强患者的认知功能、改善患者自我管理行为能力,利于患者血糖的控制,提高老年患者的生存质量,值得临床推广。 Objective:To explore the application value of computer-assisted remote cognitive training in elderly type2diabetes mellitus(T2DM)patients with mild cognitive impairment(MCI).Method:A total of80elderly T2DM patients with MCI treated from January2016to February2017in our hospital were selected and randomly divided into control group and experimental group according to the random number table method.The control group took the group education;the experimental group took the individualized integrated management through the computer-assisted remote cognitive training.The cognitive function,selfmanagement ability,blood glucose(glycosylated hemoglobin)control conditions and quality of life were evaluated.Result:The scores of Montreal cognitive assessment(MoCA)and mini-mental state examination(MMSE)for two groups were not statistically significant before intervention(P>0.05);after intervention,the scores of MoCA and MMSE in the experimental group were higher than those in the control group(P<0.05).The self-management ability in the experimental group was higher than that in the control group(P<0.05).The glycosylated hemoglobin contents in the experimental group were lower than those in the control group(P<0.05).Before intervention,the quality of life scores for two groups were not significantly different(P>0.05);after3months of intervention,the quality of life scores in the experimental group were higher than those in the control group(P<0.05).Conclusion:The computer-assisted remote cognitive training can improve the patients'cognitive function and self-management ability,control the blood glucose and increase the quality of life.It is worthy of clinical promotion.
