摘要
目的观察并比较降钙素原(procalcitonin,PCT)、超敏C-反应蛋白(high sensitive C-reactive pro,hs-CRP)对糖尿病足混合感染与多重耐药感染的敏感性。方法选择本院内分泌科2013年6月~2017年10月期间住院治疗的糖尿病足感染分泌物培养阳性患者129例,根据是否混合感染分为单一细菌感染组(SI组,91例)和混合细菌感染组(MI组,38例),根据是否多重耐药感染分为糖尿病足非多重耐药感染组(NMRI组,88例)和糖尿病足多重耐药感染组(MRI组,41例,其中单一细菌感染组19例,混合细菌感染组22例)。比较PCT、hs-CRP在各组别的变化。结果糖尿病足混合细菌感染组患者的hs-CRP高于单一细菌感染组(P<0.05),PCT水平显著高于单一细菌感染组(P<0.01);糖尿病足多重耐药感染组hs-CRP高于糖尿病足非多重耐药感染组(P<0.05),PCT水平显著高于非多重耐药感染组(P<0.01);混合细菌感染伴多重耐药组PCT水平高于单一细菌感染伴多重耐药组(P<0.05)。结论降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)对糖尿病足混合细菌感染和多重耐药感染具有一定的相关性,与超敏C-反应蛋白比较,降钙素原对糖尿病足混合细菌感染和多重耐药感染的预测更敏感。
Objective To observe and compare the sensitivity of procalcitonin(PCT)and high sensitive C-reactive pro(hs-CRP)in mixed infection and multidrug-resistant infection of diabetic foot.Methods A total of129patients with positive secretion culture of diabetic foot infection in the Department of Endocrinology from June2013to October2017were enrolled.The patients were divided into single bacterial infection group(SI group,91cases)and mixed bacterial infection according to whether the infection was mixed infection.According to whether the infection was multi-drug resistant infection,the patients were divided into diabetic foot non-multi-drug resistant infection group(NMRI group,88cases)and diabetic foot multi-drug resistant infection group(MRI group,41cases,19patients among them were in the single bacterial infection group and22patients were in the mixed bacterial infection group).The changes in PCT and hs-CRP in each group were compared.Results The hs-CRP of the diabetic foot mixed bacterial infection group was higher than that of the single bacterial infection group(P<0.05),and the PCT level in the diabetic foot mixed bacterial infection group was significantly higher than that of the single bacterial infection group(P<0.01).The hs-CRP in the diabetic foot multi-drug resistant group was higher than that of the diabetic foot non-multi-drug resistant group(P<0.05),and the PCT level in the multi-drug resistant group was significantly higher than that of the non-multi-drug resistant group(P<0.01).The PCT level in mixed bacterial infection combined with the multi-drug group was higher than that of single bacterial infection with multiple drug resistance group(P<0.05).Conclusion Procalcitonin(PCT)has a certain correlation with hypersensitive C-reactive protein(hs-CRP)in diabetic foot mixed bacterial infection and multi-drug resistant infection.Compared with hypersensitive C-reactive protein(hs-CRP),procalcitonin(PCT)is more sensitive to the prediction of mixed bacterial infections and multi-drug resistant infections in diabetic foot.
