危重症患者液体出入量记录存在的问题及对策

Problems and Countermeasures in Recording Fluid Volume in Critical Patients

目的对危重症患者液体出入量记录存在的问题进行分析,提出相应对策,使得出入量记录的准确性得以提高。方法分析67例患者护理监护单中的24 h液体出入量记录情况,总结液体出入量记录存在的具体错误情况。结果 316张护理记录单中,出入量记录错误的护理记录单214张,共有508例次的错误,其中入量错误167例次,出量错误341例次。结论为了使出入量记录的准确性得以提高,必须要让护理人员接受有关液体出入量概念的相关培训,完善出入量记录内容,按要求总结出入量记录,杜绝不易测量出入量项目的漏记现象,使用标准的量具、量表或采用称重法,使得护士的业务素质及法律意识得以提高。

Objective To analyze the problems existing in the recording of fluid inflow and outflow in critical patients,and put forward corresponding countermeasures,so as to improve the accuracy of recording of outflow and outflow.Methods The24-hour liquid intake and intake records of67patients in nursing care sheet were analyzed,and the specific errors in liquid intake and intake records were summarized.Results Among the316nursing records,214were incorrectly recorded,with508errors,of which167were incorrectly recorded and341were incorrectly recorded.Conclusion In order to make out which greatly improved the accuracy of the recording,must make the concept of nursing staff to accept the liquid intake related training,improve the discrepancy quantity record contents,summarizes the discrepancy quantity records as required,put an end to is not easy to measure in and out of the project amount of leakage phenomenon,using the standard measuring tool,scale,or use weighing method,makes the nurses'professional quality and legal consciousness to improve.
