健脾益肠方治疗轻中度脾虚型特应性皮炎临床研究

Clinical research of jianpi yichang formula in treatment of mild and moderate atopic dermatitis of spleen deficiency

目的研究与观察健脾益肠方对轻中度脾虚型特应性皮炎患者的疗效。方法选取2017年2月-12月期间皮肤科门诊64例脾虚型轻中度特应性皮炎患者为研究对象,将其随机分为对照组和观察组,每组32例。对照组采用依巴斯汀片口服联合甘草油外用治疗,观察组则采用健脾益肠方口服联合甘草油外用治疗。比较两组患者治疗前后的症状体征评分、瘙痒及影响睡眠评分、临床总有效率以及相关血液指标;在治疗结束后随访3个月,每个月随访1次,观察复发情况。结果治疗4周后,观察组临床特征评分及SCORAD总评分均显著低于对照组(P <0. 05),差异有统计学意义,而两组瘙痒及影响睡眠评分及相关血液指标比较,差异无统计学意义(P> 0. 05);观察组临床总有效率显著高于对照组(P <0. 05),差异有统计学意义。随访3个月后,观察组临床特征评分、瘙痒及影响睡眠评分以及过敏相关血液指标均低于对照组(P <0. 05),差异有统计学意义。结论健脾益肠方治疗轻中度脾虚型特应性皮炎疗效确切,安全性良好,且与口服抗组胺药相比具有停药后不易复发的优势,因此在脾虚型特应性皮炎患者中的应用价值较高。

Objective To explore the therapeutic effects of jianpi yichang formula in treatment of mild and moderate atopic dermatitis of spleen deficiency.Methods From February to December2017,64outpatients of mild and moderate atopic dermatitis of spleen deficiency were collected as the subjects from the Department of Dermatology.They were randomized into a control group and an observation group,32cases in each one.In the control group,the oral administration of ebastine tablets and the external application of licorice oil were adopted.In the observation group,the oral administration of jianpi yichang formula and the external application of licorice oil were used.Before and after treatment,the scores of symptoms and physical signs,the scores of pruritus and sleep,the total clinical effects as well as the relevant blood indexes were compared in the two groups.Additionally,the follow-up was given once every one month,for3months consecutively and the recurrence was observed.Results In4weeks of treatment,the clinical characteristic scores and SCORAD total score in the observation group were lower significantly than the control group,indicating the significant difference(P<0.05).The differences were not significant in the scores of pruritus and sleep as well as the relevant blood indexes(P>0.05).The total effective rate in the observation group was higher significantly than the control group(P<0.05).After3-month follow-up,the clinical characteristic scores,the scores of pruritus and sleep as well as the relevant blood indexes in the observation group were all lower than the control group,indicating the significant differences(P<0.05).Conclusion Jianpi yichang formula achieves the definite effects on mild and moderate atopic dermatitis of spleen deficiency and is good in safety.Compared with the oral administration of antihistamine drug,the herbal medicine presents the advantages on preventing from recurrence after drug discontinuance.Hence,this formula is highly valuable in treatment of atopic dermatitis.
