骨水泥混合异体骨粉在四肢骨肿瘤骨缺损重建中的应用效果

摘要 目的探讨骨水泥混合异体骨粉在四肢骨肿瘤刮除术后骨缺损重建中应用的临床效果。方法收集2011年1月至2017年10月接受骨水泥混合异体骨粉重建缺损且临床资料完整的62例四肢骨肿瘤患者,其中单纯骨水泥混合异体骨粉重建13例,联合内固定59例。根据骨肿瘤保肢MSTS93功能评分系统评价患肢临床功能并随访并发症和复发情况。结果截止于2018年6月,62例患者的中位随访时间为34. 3个月(范围:9~84个月)。MSTS93评分11~30分,其中优59例,良1例,中1例,差1例,优良率为96. 8%(60/62)。术后感染2例,复发1例,全组无1例发生无骨水泥松动或重建部位周围骨折。结论骨水泥混合异体骨粉对于四肢骨肿瘤刮除术后骨缺损重建是一种安全、经济、有效的方法,值得临床推广。 Objective To explore the clinical effects of bone cement mixed with allogeneic bone powder in the reconstruction of bone defects after tumor curettage of limb bones.Methods From January2011to October2017,62patients with complete data and follow-up with bone tumors of the extremities were reviewed.All patients were treated with bone cement mixed with allogeneic bone powder to reconstruct bone defects.Among them,13cases were reconstructed with bone cement and allograft bone graft,and59cases were combined with internal fixation.According to the bone tumor salvage MSTS93functional scoring system,the clinical function,complications and recurrence of the limbs were evaluated.Results To June2018,the follow-up ranged from9to84months with the median of34.3months.MSTS93score was11to30points,of which59were excellent,1good,1fair and1poor.The excellent and good rate was96.8%(60/62).There were2cases of infection and1case recurred.No bone cement loosening or fractures around the reconstruction site were found in the group.Conclusion Bone cement mixed with allogeneic bone powder is a safe,economical and effective method for reconstruction of bone defects after tumor curettage of limb bones.It is worthy of clinical promotion.
