三种常见类型胃息肉的分布及其与幽门螺杆菌感染的关系

Distribution of Three Common Types of Gastric Polyps and Their Correlation With Helicobacter pylori Infection

背景:胃息肉为胃癌前病变,幽门螺杆菌(Hp)感染在其发生过程中起有重要作用。目的:探讨三种常见类型胃息肉在胃内的分布情况及其与Hp感染的关系。方法:回顾性分析2015年12月—2018年2月徐州医科大学附属医院收治的胃息肉患者的临床资料,比较增生性息肉、胃底腺息肉和炎症性息肉的Hp阳性率。结果:共纳入胃息肉患者464例,男女之比为0. 31∶1。三种常见类型胃息肉主要分布于胃体、胃底和胃窦。Hp阳性率与息肉类型、息肉部位有关(P <0. 05),而与患者性别、年龄无关(P> 0. 05)。胃窦与非胃窦部位之间增生性息肉的Hp阳性率相比差异有统计学意义(P <0. 05),而炎症性息肉Hp阳性率无明显差异(P> 0. 05)。结论:女性胃息肉患者多于男性,胃息肉主要位于胃体、胃底和胃窦,Hp阳性率与患者性别和年龄无关,增生性息肉和炎症性息肉的Hp阳性率均高于胃底腺息肉。

Background:Gastric polyps are recognized as precancerous lesions of gastric cancer,and Helicobacter pylori(Hp)infection plays an important role in the pathogenesis of gastric polyps.Aims:To explore the distribution of three common types of gastric polyps and their relationship with Hp infection.Methods:Clinical data of gastric polyps patients from Dec.2015to Feb.2018at the Affiliated Hospital of Xuzhou Medical University were retrospectively analyzed,and Hp positivity rate was compared among hyperplastic polyps,gastric fundic gland polyps and inflammatory polyps.Results:A total of464patients with gastric polyps were included in this study,and ratio of male to female was0.31∶1.The three common types of gastric polyps were mainly located in the gastric body,fundus and antrum.Hp positivity rate was not related to gender,age of the patients(P>0.05),but related to type of polyps and location of polyps(P<0.05).There was significant difference in Hp positivity rate of hyperplastic polyps between gastric antrum and non-gastric antrum(P<0.05),however,there was no difference in inflammatory polyps(P>0.05).Conclusions:The number of female gastric polyps patients is higher than that of male patients.Gastric polyps are mainly located in gastric body,fundus and antrum.The positivity rate of Hp is not related to gender and age.Hp positivity rate of hyperplastic polyps and inflammatory polyps are higher than that of gastric fundic gland polyps.
