IABP在老年急性心肌梗死并心源性休克治疗中的运用分析

The application of IABP in the treatment of acute myocardial infarction and cardiogenic shock in the elderly

目的探讨在老年急性心肌梗死(AMI)并发心源性休克治疗中,主动脉球囊反搏(IABP)的临床运用效果。方法选取本院接诊IABP治疗AMI并发心源性休克患者32例作为IABP组,另选取同期非IABP治疗患者32例作为对照组,观察两组心率、平均动脉压、尿量变化以及CI、LVEF、NT-proBNP、SCr变化,统计30 d死亡率。结果 IABP组术后平均动脉压、尿量均明显高于对照组,心率低于对照组(P<0.05);IABP组术后LVER、CI均明显高于对照组,BNP、SCr低于对照组(P<0.05);IABP组30 d死亡率为21.88%,对照组死亡率为50.00%(P<0.05)。结论通过使用IABP可在短时间内较好的实现对AMI并心源性休克患者血流动力学、体征的稳定,同时实现对30 d死亡率的降低。

Objective To investigate the clinical effect of aortic balloon counterpulsation(IABP)in the treatment of cardiogenic shock in elderly patients with acute myocardial infarction(AMI).Methods In our hospital IABP treatment of AMI complicated with cardiogenic shock in32patients were selected as IABP group,non IABP patients,32cases as control group,observe two groups of heart rate,mean arterial pressure,urine volume,LVEF,CI change and NT-proBNP、SCrchange,30d mortality statistics.Results IABP group of postoperative heart rate,mean arterial pressure and urine volume were significantly higher than the control group(P<0.05);IABP group after LVER and CI were significantly higher than the control group,BNP、SCr was lower than the control group(P<0.05);IABP group30d mortality rate was21.88%,the control group mortality was50.00%(P<0.05).Conclusion By using IABP,we can achieve better stability of hemodynamics and signs in patients with AMI and cardiogenic shock in a short time,and at the same time,reduce30d mortality.
