阻塞性睡眠呼吸暂停与原发性高血压患者对比剂肾病发生的关系

Correlation between obstructive sleep apnea and contrast induced nephropathy in patients with hypertension

目的探讨阻塞性睡眠呼吸暂停(obstructive sleep apnea,OSA)与原发性高血压(高血压)患者对比剂肾病(contrast induced nephropathy,CIN)发生的相关性。方法收集2013年6月至2017年3月入住深圳市盐田区人民医院及福建医科大学附属龙岩市第一医院,诊断为高血压并行冠状动脉介入检查及治疗的患者387例,其中OSA患者118例(OSA组),无OSA患者269例(非OSA组),对比两组CIN发病率的差别,并分析高血压患者CIN发病率与OSA严重程度的相关性。结果高血压合并OSA组CIN的发病率显著高于高血压无OSA组,差异有统计学意义(21.19%vs. 11.90%,P<0.05);高血压合并重度OSA组CIN的发病率显著高于高血压合并轻中度OSA组,差异有统计学意义(30.36%vs. 12.90%,P<0.05)。结论 OSA是高血压患者发生CIN的危险因素,随着OSA严重程度增加,患者CIN发病率越高。

Objectives To explore the correlation between obstructive sleep apnea(OSA)and contrast induced nephropathy(CIN)in patients with hypertension.Methods Totally387patients with hypertension and performed percutaneous coronary intervention(PCI)from June2013to March2017in The People′s Hospital of Yantian Distric in Shenzhen City and The First Hospital of Longyan City Affiliated to Fujian Medical University were collected.Among the patients,118patients were with OSA(OSA group)and269patients without OSA(non OSA group).The difference of incidence of CIN between the two groups were compared.Relationship between severity of OSA and incidence of CIN in patients with hypertension were analyzed.Results Incidence of CIN in patients with hypertension and OSA was signifi?cantly higher than that in patients with hypertension without OSA(21.19%vs.11.90%,P<0.05);Incidence of CIN in patients with hypertension and severe OSA was significantly higher than that in patients with hypertension and mild or moderate OSA(30.36%vs.12.90%,P<0.05).Conclusions OSA may be a risk factor in hypertension patients with CIN.As OSA is more serious,incidence of CIN may be higher.
