微小RNA-195在高血压左心室肥厚诊断中的价值 被引量:3

摘要 目的探讨微小RNA-195(microRNA-195,miR-195)在高血压左心室肥厚诊断中的价值。方法选择2016年6月至2017年10月珠海市人民医院急诊科门诊的单纯高血压患者180例纳入观察组,另选择同期在我院体检的健康者100名纳入对照组。两组均行诊室血压、动态血压检测和超声心动图检查,并应用反转录聚合酶链反应(reverse transcription-polymerase chain reaction,RT-PCR)技术检测两组血浆中miR-195的表达水平。采用Spearman相关分析法分析miR-195表达水平与与血压参数、左心室质量指数的相关性。结果与对照组比较,观察组左心房内径、左心房舒张末期内径、左心室质量、左心室质量指数、左心室后壁厚度、室间隔厚度、24 h平均收缩压、24 h平均舒张压、白天收缩压、白天舒张压、诊室收缩压、诊室舒张压、夜间收缩压、夜间舒张压及miR-195表达水平均明显较高,且差异有统计学意义(P<0.05)。Spearman相关性结果提示,miR-195表达水平与左心室质量、左心室质量指数、白天收缩压、白天舒张压、24 h平均收缩压、24 h平均舒张压、诊室收缩压及诊室舒张压均呈正相关性(P<0.05)。观察组中左心室肥厚者血浆miR-195表达水平显著高于非左心室肥厚者,且差异有统计学意义(33.94±2.47 vs. 31.04±2.13,t=8.362,P=0.000)。结论 MiR-195高表达于高血压左心室肥厚患者的外周循环血浆中,且miR-195的表达水平与左心室质量指数呈正相关性,其可能成为诊断高血压左心室肥厚的潜在标志物。 Objectives To investigate the value of microRNA-195(miR-195)in the diagnosis of left ventricular hypertrophy of hypertension.Methods Totally180patients with simple hypertension admitted to Zhuhai People′s Hospital from June2016to October2017were enrolled in observation group.Another100healthy-check people in our hospital were enrolled in control group during the same period.The two groups were measured with clinic blood pressure,ambulatory blood pressure and were checked by echocardiography.Expression of miR-195in plasma was detected by reverse transcription-polymerase chain reaction(RT-PCR).Spearman correlation analysis was used to analyze the correlation between miR-195expression and blood pressure parameters and left ventricular mass index.Results Compared with control group,left atrial diameter,left atrium diastolic diameter,left ventricular mass,left ventricular mass index,left ventricular posterior wall thickness,interventricular septum thickness,24-hour mean systolic blood pressure,24-hour mean diastolic blood pressure,diastolic blood pressure at daytime,systolic blood pressure in clinic,diastolic blood pressure in clinic,diastolic blood pressure at night,systolic blood pressure at night and expression of MiR-195were all significantly higher in observation group(P<0.05).Spearman′s correlation analysis indicated that expression of miR-195was positively correlated with left ventricular mass,left ventricular mass index,daytime systolic blood pressure,daytime diastolic blood pressure,24-hour mean systolic blood pressure,24-hour mean diastolic blood pressure,systolic blood pressure and diastolic blood pressure in clinic(P<0.05).Plasma miR-195expression in patients with left ventricular hypertrophy in observation group was significantly higher than that in patients with non-left ventricular hypertrophy,and the difference was statistically significant(33.94±2.47vs.31.04±2.13,t=8.362,P=0.000).Conclusions MiR-195is over expressed in peripheral blood of patients with hypertensive left ventricular hypertrophy,and expression of miR-195is positively correlated with left ventricular mass index.MiR-195may be a potential marker of hypertensive left ventricular hypertrophy.
