腹腔镜与内镜联合治疗对胃部肿瘤患者术中及术后治疗效果的影响

Effect of laparoscopy combined with endoscopy on intraoperative and postoperative efficacies of patients with gastric tumors

目的研究腹腔镜联合内镜治疗对胃部肿瘤患者术中和术后治疗效果的影响。方法回顾性分析42例胃部肿瘤患者的临床资料,根据治疗方法不同分为对照组(传统开腹手术治疗)21例和观察组(腹腔镜联合内镜手术治疗)21例。比较两组患者的一般手术指标(术中出血量、手术时间、术后恢复时间)、复发风险、术后并发症(腹腔感染、切口感染和术后穿孔)的发生情况和术后6个月的复发率。结果观察组患者术中出血量为(45.62±12.37)ml,明显少于对照组的(110.47±32.56)ml,差异有统计学意义(P﹤0.01);手术时间、术后恢复时间分别为(69.25±10.38)min、(6.54±2.15)d,均明显短于对照组的(80.56±12.44)min、(10.36±2.47)d,差异均有统计学意义(P﹤0.01)。两组患者复发风险、并发症的发生率和复发率比较,差异均无统计学意义(P﹥0.05)。结论腹腔镜联合内镜治疗胃部肿瘤,创伤小,安全方便,临床效果较好。

Objective To study the effect of laparoscopy combined with endoscopy on the intraoperative and postoperative efficacies of patients with gastric tumors.Method The clinical data of42patients with gastric tumor were retrospectively analyzed.According to different treatment methods,21patients treated with traditional open surgery assigned as control group and21patients treated with laparoscopic combined with endoscopic surgery assigned as observation group.The general surgical parameters(intraoperative blood loss,operation time,postoperative recovery time),risk of recurrence,incidences of postoperative complications(abdominal infection,incision infection,and postoperative perforation)and recurrence rate at6months after surgery were compared between the two groups.Result The intraoperative blood loss of the observation group was(45.62±12.37)ml,which was significantly less than that(110.47±32.56)ml of the control group,the difference was statistically significant(P<0.01).The operation time and postoperative recovery time of observation group were(69.25±10.38)min and(6.54±2.15)d,respectively,which were significantly shorter than those(80.56±12.44)min and(10.36±2.47)d of control group,the difference was statistically significant(P<0.01).There was no significant difference in the risk of recurrence,the incidences of complications and the recurrence rate between the two groups(P>0.05).Conclusion Laparoscopy combined with endoscopy for the treatment of gastric tumors is with less surgical trauma,safety and convenience,and good clinical efficacy.
