miRNA-17/92a基因簇、P1NP和β-CTX在诊断绝经后骨质疏松症中的应用价值 被引量:9

摘要 目的探讨血清miRNA-17/92a基因簇、骨Ⅰ型前胶原氨基端延长肽(P1NP)和Ⅰ型胶原羧基端肽β特殊序列(β-CTX)测定在绝经后骨质疏松症(PMOP)中的应用价值。方法选取150例绝经后女性作为研究对象,根据骨密度T值将全部研究对象分为OP组、骨量降低组和对照组。检测全部研究对象的miRNA-17/92a基因簇表达水平和骨代谢指标P1NP、β-CTX的变化。比较OP组、骨量降低组和对照组的血清miRNA-17/92a、P1NP和β-CTX水平。分析血清miRNA-17/92a与患者年龄、绝经后时间、BMI、P1NP、β-CTX的相关性。通过ROC曲线分析血浆中miRNA-17/92a基因簇含量、P1NP、β-CTX对PMOP的诊断灵敏度和特异度。结果 OP组的血清miRNA-17/92a基因簇含量显著低于骨量降低组和对照组,OP组的血清P1NP、β-CTX水平显著高于骨量降低组和对照组。miRNA-17/92a基因簇含量与年龄、绝经后时间、P1NP和β-CTX呈显著负相关关系(P <0. 05),与BMI无显著相关性。血清miRNA-17/92a、P1NP、β-CTX指标联合诊断PMOP效果最佳,AUC、灵敏度和特异度分别为0. 85、93. 5%和90. 2%。结论 miRNA-17/92a、P1NP、β-CTX三者联合检测对PMOP的诊断价值高于单独检测。 Objective To study the value of miRNA-17/92a gene cluster,PINP andβ-CTX in the diagnosis of postmenopausal osteoporosis.Methods Totally150postmenopausal women were selected as the study subjects.According to the T value of bone mineral density,all the subjects were divided into OP group,low-bone mass group and control group.The expression level of miRNA-17/92a gene cluster and the changes of bone metabolism markers P1NP andβ-CTX were detected in all subjects.The serum miRNA-17/92a,P1NP andβ-CTX levels in OP group,low-bone mass group and control group were compared to analyze the correlation between serum miRNA-17/92a and age,postmenopausal time,BMI,PlNP and(3-CTX.The diagnostic sensitivity and specificity of miRNA-17/92a,P1NP andβ-CTX for PMOP were compared by ROC curve analysis.Results The level of serum miRNA-17/92a gene cluster in OP group was significantly lower than that in low-bone mass group and control group.Theβ-CTX level in OP group was significantly higher than that in low-bone mass group and control group.The content of miRNA-17/92a gene cluster was negatively correlated with age,postmenopausal time,PI NP andβ-CTX(P<0.05),but not with BMI.Serum miRNA-17/92a of P1NP andβ-CTX was the best in the diagnosis of PMOP.The AUC,sensitivity and specificity were0.85,93.5%and90.2%Respectively.Conclusion The diagnostic value of combined detection of miRNA-17/92a,P1NP andβ-CTX in PMOP is higher than that in single detection.
