摘要
目的研究血必净联合抗菌药物治疗对ICU重症肺炎患者血清感染指标[C反应蛋白(CRP)、降钙素原(PCT)]、急性蛋白指标[α1抗胰蛋白酶(α1-AT)、α1酸性糖蛋白(α1-AG)、铜蓝蛋白(CER)]、应激激素指标[皮质醇(Cor)、血管紧张素Ⅰ(AngⅠ)、血管紧张素Ⅱ(AngⅡ)]的影响。方法选择2016年4月至2017年8月在我院ICU接受治疗的120例重症肺炎患者为研究对象,按随机数字表分为研究组与对照组,每组60例。对照组采用抗菌药物治疗,研究组在此基础上加用血必净进行治疗。比较两组疗效,观察两组治疗前及治疗7 d的感染、急性蛋白、应激激素等指标的变化。结果研究组总有效率高于对照组(95. 00%vs. 81. 67%,P <0. 05)。治疗前,两组患者血清CRP、PCT、α1-AT、α1-AG、CER、Cor、AngⅠ、AngⅡ水平比较差异无统计学意义(P> 0. 05);治疗7 d后,研究组上述指标水平均低于对照组(P <0. 05)。结论血必净联合抗菌药物治疗ICU重症肺炎患者的效果显著,可以有效降低患者血清感染指标、急性蛋白和应激激素指标,进而抑制患者全身感染与应激状态,控制患者的病情,值得推广应用。
Objective To study the effect of Xuebijing combined with antibacterial on infection indexes[including C-reaction protein(CRP)and procalcitonin(PCT)],acute protein indexes[alpha1antitrypsin(1-AT alpha),alpha1acid glycoprotein(alpha1-AG)and ceruloplasmin(CER)]and the stress hormone indexes[cortisol(Cor),angiotensin I(Ang I)and angiotensin II(Ang II)]of patients with severe pneumonia in ICU.Methods Totally120patients with severe pneumonia who received treatment in ICU of our hospital from April2016to August2017were collected and divided into2groups according to the random number table.Patients in control group(n=60)were given antibacterial treatment,and patients in study group(n=60)were treated with Xuebijing combined with antibacterial treatment.The efficacy of the two groups were compared,and the indexes of infection,acute protein and the stress hormone were observed.Results The total effective rate of study group was higher than that of control group(95.00%vs.81.67%<0.05).There was no significant difference in serum CRP,PCT,α1-AT,α1-AG,CER,Cor,Ang I or Ang II between the two groups(P>0.05),and the levels of above indexes in study group were lower than those of control group after7d(P<0.05).Conclusion Xuebijing combined with antibacterial has obvious effect on ICU patients with severe pneumonia,reduces the levels of indexes of serum infection,acute protein and stress hormone,and inhibits the systemic status of infection and stress,which is worthy of popularization and application.
