参附注射液联合肺保护性通气在胸科手术患者中的应用效果

Application efficacy of Shenfu Injection combined with lung protective ventilation for patients undergoing thoracic surgery

目的观察参附注射液联合肺保护性通气在胸科手术患者中的应用效果。方法将100例行胸腔镜下右肺癌根治术患者,随机分为对照组和观察组,每组50例。两组患者均给予肺保护性通气干预,观察组在麻醉诱导前30 min给予参附注射液静脉滴注。比较两组患者手术情况、氧合指数、气道阻力、术后并发症发生率及血液流变学指标。结果两组手术时间、出血量、单肺通气时间、ICU停留时间、术后住院时间比较,差异均无统计学意义(均P> 0. 05)。单肺通气2 h后,观察组患者氧合指数高于对照组,气道阻力、血液流变学各指标水平均低于对照组(均P <0. 05)。观察组术后肺部并发症发生率低于对照组(P <0. 05)。结论与单纯肺保护性通气比较,参附注射液联合肺保护性通气可更好地改善胸科手术患者的肺呼吸功能及血液高凝状态,降低术后并发症发生率。

Objective To observe the application efficacy of Shenfu Injection combined with lung protective ventilation for patients undergoing thoracic surgery.Methods One hundred patients with right lung cancer undergoing thoracoscopic radical surgery were randomly divided into control group and observation group,with50cases in each group.Both groups were given intervention with lung protective ventilation,besides,the observation group was given intravenous drip of Shenfu Injection at30minutes before anesthesia induction.The operation conditions,oxygenation index,airway resistance and incidence rate of postoperative complications,as well as the hemorheologic indices,were compared between the two groups.Results There was no statistically significant difference in the operation duration,bleeding volume,single-lung ventilation duration,ICU stay or postoperative hospital stay between the two groups(all P>0.05).After2hours of single-lung ventilation,the observation group had higher oxygenation index,lower airway resistance and lower levels of the hemorheologic indices compared with the control group(all P<0.05).The incidence rate of postoperative pulmonary complications in the observation group was lower than that in the control group(P<0.05).Conclusion Compared with lung protective ventilation alone,Shenfu Injection combined with lung protective ventilation can better improve respiratory function and hypercoagulable state as well as reduce the incidence rate of postoperative complications in the patients undergoing thoracic surgery.
