摘要
目的探讨胸部肿瘤放疗患者双手放置摆位误差与治疗床位置误差的相关性。方法选择胸段食管癌患者172例,根据放疗时双手放置方式的不同分为观察组72例与对照组100例,两组都给予三维适形放射治疗,体位固定装置为新型颈胸膜,观察组放疗时双手抱肘置于额头上,对照组放疗时双手置于身体两侧,记录摆位误差与治疗床位置误差,统计校准情况。结果观察组X、Y方向的摆位误差明显少于对照组(P <0. 05),两组Z方向的摆位误差对比无明显差异(P> 0. 05)。观察组X、Y、Z方向的校准频率分别为20. 1%、12. 9%和18. 8%,对照组分别为53. 0%、47. 8%和30. 2%,观察组都低于对照组(P <0. 05)。两组X、Y、Z方向的治疗床位置误差对比差异都无统计学意义(P> 0. 05),都无超过0. 2 mm。结论相对于双手放体侧,基于双手上举的新型颈胸膜固定方式在食管癌放疗中的应用能减少X、Y方向的摆位误差,且不会增加治疗床位置误差,也能减少放疗中校准的次数,有很好的应用价值。
Objective To investigate the correlation of the set-up error and treatment bed position error of different hands position in the esophageal cancer patients treated by the radiotherapy.Methods 172 esophageal cancer patients were divided into the 72 patients of observation group and 100 patients of control group accorded to the different hands position,two groups were treated with three-dimensional conformal radiotherapy,and the posture fixing device was the new type of cervical pleura.The observation group was used the hands elbow on the forehead during the radiotherapy,and the control group was used the hands placed on both sides of the body during the radiotherapy,recorded set-up error and treatment bed position error,the calibration rates were statistically recorded.Results The set-up error at the direction of X and Y in the observation group were significantly less than that in the control group,the difference was statistically significant(P<0.05),and there were no significant difference between the two groups in the Z direction(P>0.05).The calibration rates at the direction of X,Y and Z in the observation group were 20.1%,12.9%and 18.8%,respectively.The control group was 53.0%,47.8%and 30.2%,respectively,and the observation group was lower than that of the control group(P<0.05).Two groups of X,Y,Z direction of the treatment bed position error compared were not statistically significant(P>0.05),and the error were no more than 0.2 mm.Conclusion Compares with the hands on the side of the body,the new type of cervical pleura of hands elbow on the forehead on the application in the radiotherapy of esophageal cancer can reduce the X and Y direction of set-up error,and will not increase the treatment bed position error,but also can reduce the number of radiation calibration,so it has good application values.
