甲状腺肿瘤恶性程度与三维能量多普勒超声血管参数的相关性研究

Correlation between Malignant Degree of Thyroid Tumor and Three-dimensional Energy Doppler Ultrasonography Vascular Parameters

目的探讨甲状腺肿瘤恶性程度与三维能量多普勒超声血管参数的关系。方法 73例甲状腺癌与85例甲状腺腺瘤患者,术前运用三维能量多普勒超声检测血管形成指数(VI),分析VI值与肿瘤良恶性、恶性肿瘤病理分型、直径、TNM分期及淋巴结转移的关系。结果甲状腺癌的VI值明显大于甲状腺腺瘤(P <0. 05)。不同病理分型、不同肿瘤直径的VI值比较,差异无统计学意义(P> 0. 05); TNMⅢ～Ⅳ期的VI值明显大于Ⅰ～Ⅱ期(P <0. 05);有淋巴结转移的VI值明显大于无淋巴结转移(P <0. 05)。结论三维能量多普勒超声参数VI值能一定程度地反映甲状腺肿瘤的恶性程度及转移情况。

Objective To investigate the relationship between malignant degree of thyroid tumor and three-dimensional energy Doppler ultrasound vascular parameters.Methods 73 cases of thyroid carcinoma and 85 cases of thyroid adenoma were collected from May.2014 to Dec.2016.The blood vessel formation index(VI)was measured by three-dimensional energy Doppler ultrasonography before operation.The relationship between VI value and pathological type,diameter,TNM staging and lymph node metastasis of benign and malignant tumor were analyzed.Results The VI value of thyroid carcinoma was significantly higher than that of thyroid adenoma(P<0.05).The VI values of different pathological types and diameters of different tumors were not statistically significant(P>0.05),while the VI value of TNMⅢ~Ⅳwas obviously higher than that ofⅠ~Ⅱ(P<0.05),and the VI value of lymph node metastasis was significantly higher than that without lymph node metastasis(P<0.05).Conclusion The VI value of three-dimensional energy Doppler ultrasound can reflect the malignancy and metastasis of thyroid tumor to a certain extent.
