靶向肿瘤内高乳酸环境经肝动脉化疗栓塞术治疗肝细胞癌的临床观察

Clinical observation of TILA-TACE and TACE in treatment of unresectable hepatocellular carcinoma

目的探讨靶向肿瘤内高乳酸环境肝动脉化疗栓塞术(TILA-TACE)治疗不可切除肝细胞癌的疗效。方法回顾性分析我院2015年7月至2016年9月收治的60例肝细胞癌(HCC)患者的临床资料,按治疗方法的不同分为TILA-TACE组和TACE组,TACE组30例患者单纯行TACE治疗; TILA-TACE组30例患者在行TACE治疗的同时灌注碳酸氢钠。比较2组患者的临床疗效、安全性及术后12个月的生存率。结果 TILA-TACE组疾病控制率为86. 67%,高于TACE组的56. 67%,2组比较差异具有统计学意义(P <0. 05); TILA-TACE组术后12个月生存率为80. 00%,高于TACE组的53. 33%,2组比较差异具有统计学意义(P <0. 05); 2组治疗相关不良反应差异无统计学意义(P> 0. 05)。结论靶向肿瘤内高乳酸环境经肝动脉化疗栓塞术治疗不可切除肝癌疗效肯定,安全可靠。

Objective To explore the curative effect of targeting intratumoral lactic acidosis transcatheter arterial chemoembolization(TILA-TACE)for the treatment of unresectable hepatocellular carcinoma(HCC).Methods From July2015to September2016,a total of60patients of HCC were enrolled in this study.And these patients were divided into the TILA-TACE group and the TACE group with30cases in each group according to the different surgical methods.Patients of the TACE group were treated with TACE merely,while patients in the TILA-TACE group were treated with perfusion of sodium bicarbonate in addition to TACE.The curative effect,safety,and survival rate in12months after surgery of the two groups were compared.Results The disease control rate was86.67%in the TILA-TACE group,which was higher than56.67%in the TACE group,and the difference was statistically significant(P<0.05).The survival rate12months after surgery was80.00%in the TILA-TACE group,and it was53.33%in the TACE group,with statistically significant difference between the two groups(P>0.05).There were no statistically significant differences between the two groups in terms of related adverse reactions.Conclusion Targeting intratumoral lactic acidosis transcatheter arterial chemoembolization is effective and safe for the treatment of unresectable hepatocellular carcinoma.
