脊柱内固定术后手术部位感染的原因分析及防治措施

Causes analysis and prevention measures of surgical site infection after spinal internal fixation

目的探究脊柱内固定术后手术部位感染的原因及防治措施。方法选取我院2015年1月至2018年2月收治的500例实施脊柱内固定术的患者作为研究对象,分析其临床资料,明确导致术后手术部位感染的原因,并制定针对性的防治措施。结果 500例患者中有10例发生术后感染,发生率为2. 00%,经χ2检验分析及多因素回归分析发现,吸烟、糖尿病、抗生素使用、大小便失禁、手术时间和节段、脊柱创伤指数均为影响术后感染的独立危险因素(P <0. 05)。结论脊柱内固定术临床治疗和护理中,应针对影响术后手术部位感染的独立因素,依据患者的病情和个体特征,制定针对性的预防措施,利用抗生素和清创灌洗引流等方法实施有效治疗。

Objective To study the causes and prevention measures of surgical site infection after spinal internal fixation.Methods A total of500patients who were treated in our hospital from January2015to February2018were selected as the research object.The clinical data of these patients were analyzed to clear the causes of the postoperative infection and to make preventive measures.Results There were10cases of postoperative infection among the500patients with an infection rate of2.00%.According to the χ^2 test analysis and multiple regression analysis,it was found that smoking,diabetes,antibiotic use,incontinence,operation time and segment,and spinal trauma index were independent factors affecting postoperative infection(P<0.05).Conclusion In the treatment and nursing of spinal internal fixation,targeted preventive measures should be made based on the patient's condition and individual characteristics so as to cope with the independent factors that affect the postoperative infection.Antibiotics,debridement and drainage could be the effective treatment.
