摘要
目的研究骨关节炎患者肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白介素-17(interleukin-17,IL-17)、软骨寡聚基质蛋白(cartilage oligomeric matrix protein,COMP)、Ⅰ型血小板结合蛋白基序的解聚蛋白样金属蛋白酶-7(a disint egrin and metalloprotease with thrombospondin motifs-7,ADAMTS-7)及mRNA表达水平。方法选择2012年1月至2017年12月本院收治的100例骨关节炎患者为研究对象,根据患者的病情程度将其分为轻度病变组、中度病变组、重度病变组。统计三组患者基本临床资料,采用酶联免疫吸附测定检测患者关节液中TNF-α、IL-17及COMP水平,采用逆转录聚合酶链反应检测患者滑膜组织中ADAMTS-7 mRNA表达情况,免疫组化法检测患者滑膜组织中ADAMTS-7表达水平,分析骨关节炎病变程度与上述指标表达水平的相关性。结果重度病变组患者年龄均明显大于轻度病变组和中度病变组(P_均<0.05),病程均明显长于轻度病变组和中度病变组(P_均<0.05),Lysholm评分均明显低于轻度病变组和中度病变组(P_均<0.05)。重度病变组患者关节液中TNF-α、IL-17及COMP水平均显著高于中度病变组和轻度病变组(P_均<0.05),中度病变组患者TNF-α、IL-17及COMP水平均显著高于轻度病变组(P_均<0.05)。ADAMTS-7蛋白表达强度和ADAMTS-7 mRNA表达水平在不同病变程度患者的滑膜组织中表达强度比较:重度病变组>中度病变组>轻度病变组。骨关节炎病变程度与滑膜组织ADAMTS-7蛋白、ADAMTS-7 mRNA表达及关节液中COMP、IL-17及TNF-α表达均呈正相关(P_均<0.05),与Lysholm评分呈负相关(P<0.05)。结论骨关节炎患者TNF-α、IL-17、COMP、ADAMTS-7及m RNA表达水平与病情严重程度具有相关性。
Objective To investigate the expression levels of tumor necrosis factor-α(TNF-α),interleukin-17(IL-17),cartilage oligomeric matrix protein(COMP),a disintegrin and metalloprotease with thrombospondin motifs-7(ADAMTS-7)and mRNA of patients with osteoarthritis.Method100cases of osteoarthritis patients admitted to our hospital from January2012to December2017were selected as the research objects,and they were divided into mild lesion group,moderate lesion group and severe lesion group according to the degree of disease.The basic clinical data of the three groups were analyzed,the expression levels of TNF-α,IL-17and COMP levels in synovial fluid by enzyme-linked immunosorbent assay,and ADAMT-7mRNA expression in synovial membrane tissue was determined by reverse transcription-polymerase chain reaction.ADAMT-7expression level in synovial membrane tissue was determined by immunohistochemistry to analyze the correlation between the degree of osteoarthritis lesions and the expression levels of the above indicators.Result The age of patients in severe lesion group was significantly higher than that in mild lesion group and moderate lesion group(Pall<0.05),the course of disease was significantly longer than that in other two groups(Pall<0.05),Lysholm score was significantly lower than that in other two groups(Pall<0.05),TNF-α,IL-17and COMP levels were significantly higher than those in other two groups(Pall<0.05).The levels of TNF-α,IL-17and COMP in severe lesion group were significantly higher in moderate lesion group than those in mild lesion group(Pall<0.05),the levels of TNF-α,IL-17and COMP in moderate lesion group were significantly higher than those in mild lesion group(Pall<0.05).Comparison of ADAMT-7protein expression intensity and ADAMT-7mRNA expression level in synovial membrane tissue of patients with different degrees of lesion:severe lesion group>moderate lesion group>mild lesion group.The degree of osteoarthritis was positively correlated with expression level of ADAMT-7protein,ADAMT-7mRNA,COMP,IL-17and TNF-α(Pall<0.05),and negatively correlated with Lysholm score(P<0.05).Conclusion The expression levels of TNF-α,IL-17,COMP,ADAMTS-7and mRNA of osteoarthritis patients were correlated with the severity of the disease.
