我院门诊不合理处方干预和改进措施

Unreasonable prescription intervention and improvement measures in our hospital

目的对本院门诊处方不合理情况进行干预,以促进临床合理用药,充分发挥药师在临床治疗中的作用。方法对820份门诊干预的不合理处方进行统计分析,并提出整改措施。结果处方干预涉及的临床科室主要集中在儿科、妇科、计划生育和产科,构成比分别为36.8%、24.1%、13.4%、9.1%,采纳率分别为90.0%、85.9%、84.5%、85.3%。处方不合理及不适宜情况主要为用法、用量不适宜及书写不规范或录入错误、适应证不适宜,构成比分别为39.8%、13.8%、12.0%。结论药师干预处方后可以提高处方质量,促进临床用药规范性,预防潜在的用药问题,对于提高医疗质量、改善医患关系起到积极的作用。

Objective To intervene in the unreasonable situation of outpatient prescriptions in our hospital so as to promote rational drug use in clinic and give full play to the role of pharmacists in clinical treatment.Methods A total of 820 unreasonable prescriptions for out-patient intervention were statistically analyzed,and rectification measures were put forward.Results The clinical departments involved in prescription intervention mainly concentrated in pediatrics,gynecology,family planning and obstetrics,with the proportion of 36.8%,24.1%,13.4%and 9.1%respectively and adoption rates of 90.0%,85.9%,84.5%and 85.3%respectively.The unreasonable and unsuitable prescriptions were mainly usage,inappropriate dosage,improper writing or incorrect entry,and inappropriate indications,and the constituent ratios were 39.8%,13.8%and 12.0%respectively.Conclusion Pharmacists’intervention in prescriptions can improve the quality of prescriptions,promote the standardization of clinical medication,prevent potential drug problems,and this method plays an active role in improving medical quality and improving doctor-patient relationship.
