DAMPs/IL-33、PCT联合检测在肺炎鉴别诊断中的应用研究

目的研究DAMPs/IL-33血清浓度变化与各种肺炎的相关性及联合检测DAMPs/IL-33、PCT在肺炎中的鉴别诊断价值。方法收集2016年1月-2018年3月广东省珠海市香洲区人民医院收治的病毒性肺炎患者、支原体肺炎患者、细菌性肺炎患者和健康人群血液标本,共计132例,其中细菌组22例、支原体组20例、巨细胞病毒组25例、呼吸道合胞病毒组25例、腺病毒组20例与对照组20例。分析各组白细胞介素-33(IL-33)、降钙素原(PCT)水平。结果细菌组血清PCT浓度分别高于其他各组,具有统计学差异;巨细胞病毒组血清PCT浓度低于细菌组,但高于其他组,具有统计学差异。细菌组IL-33血清浓度高于其他组,各病毒组IL-33血清浓度低于细菌组,但高于支原体组及对照组,具有统计学差异。结论外周血IL-33、PCT联合检测在鉴别诊断各种肺炎中有一定的诊断价值。

Objective To study the correlation between serum concentration of DAMPs/IL-33 and various pneumonia,and to jointly detect the differential diagnosis value of DAMPs/IL-33 and PCT in pneumonia.Methods Blood samples from patients with viral pneumonia,mycoplasmal pneumonia,bacterial pneumonia and normal patients from Xiangzhou District People's Hospital of Zhuhai City,Guangdong Province from January 2016 to March 2018 were collected.A total of 132 cases were included.There were 20 cases of mycoplasma group,25 cases of cytomegalovirus group,25 cases of respiratory syncytial virus group,20 cases of adenovirus group and 20 cases of control group.The levels of interleukin-33(IL-33)and procalcitonin(PCT)were analyzed.Results The serum PCT concentration in bacterial group was higher than those of other groups,and the serum PCT concentration in cytomegalovirus group was lower than that in bacterial group,but higher than those in other groups.The serum concentration of IL-33 in bacteria group was higher than those in other groups,and the serum concentration of IL-33 in each virus group was lower than that in bacteria group,but higher than that in mycoplasma group and normal control group.Conclusion The combined detection of IL-33,PCT in peripheral blood has certain diagnostic value in differential diagnosis of various pneumonia.
