摘要
目的探讨基层医院缺血性脑卒中绿色通道的优化措施对急性缺血性脑卒中(AIS)患者静脉溶栓治疗的影响。方法选取2014年7月~2015年7月我院绿色通道优化前收治的130例AIS患者作为对照组,2015年8月~2016年7月绿色通道优化后收治的130例AIS患者作为观察组,分别采用优化前及优化后的绿色通道及溶栓流程。比较两组患者自就诊至接受溶栓治疗的时间(DNT)、DNT≤60 min达标率、溶栓率及脑卒中神经功能缺损量表评分情况。结果观察组患者的DNT短于对照组,DNT≤60 min达标率高于对照组,差异有统计学意义(P<0.05)。观察组患者的溶栓率为70.77%,高于对照组的51.54%,差异有统计学意义(P<0.05)。观察组患者的神经功能恢复程度评分有效率高于对照组,差异有统计学意义(P<0.05)。结论基层医院缺血性脑卒中绿色通道优化措施可有效缩短DNT,提高溶栓率,有利于AIS患者的治疗。
Objective To investigate the effect of green passage optimization measures on intravenous thrombolytic therapy in patients with acute ischemic stroke(AIS).Methods Altogether 130 patients with AIS who were treated in our hospital from July 2014 to July 2015 before green passage optimization were taken as the control group,and 130 patients who were treated after green passage optimization from August 2015 to July 2016 were selected as the observation group.The green passage and thrombolytic procedures before and after optimization were adopted respectively for control group and observation group.The door to needle time(DNT),qualified rate(DNT≤60 min),thrombolysis rate and the score of stroke neurological deficit scale were compared between the two groups.Results The DNT of the observation group was shorter than that of the control group,and its qualified rate(DNT≤60 min)was higher than that of the control group,with statistically significant differences(P<0.05).The thrombolysis rate of the observation group(70.77%)was higher than that of the control group(51.54%),with statistically significant difference(P<0.05).The effectiveness rate of neurological function recovery score in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).Conclusion The green passage optimization measures for ischemic stroke in primary hospitals can effectively shorten DNT,increase the rate of thrombolysis,and facilitate the treatment of patients with AIS.
