饮食不当致1例心脏术后抗凝INR异常

摘要 通过药学门诊咨询,临床药师对1例心脏瓣膜置换术后患者服用华法林期间出现国际标准化比值(INR)持续偏低的原因进行分析,考虑可能的原因为患者长期饮用豆浆及食用南瓜,并给予患者适当的饮食调整建议,使患者的INR恢复到治疗范围内,此案例为临床药师开展药学服务提供参考。 Clinical pharmacists participated in consultation and analyzed the causes of persistent low INR during warfarin administration in a patient after heart valve replacement.Based on analysis,pharmacists considered that the long-term consumption of soymilk and pumpkins may lead to the INR abnormality and gave the patient appropriate dietary advices.INR gradually turned to normal with lifestyle and dietary changes.Clinical pharmacists’analysis on the case would contribute to future improvement.
