基于连环替代的我国次均医疗费用影响因素分析

Analysis of the Influencing Factors of Medical Expenditure Per Patient in China Based on Chain Substitution Method

目的研究我国医院门诊和住院次均医疗费用的主要影响因素,为控制次均医疗费用的增长提供理论依据。方法收集2010—2016年全国医院的医疗费用数据,采用连环替代法分析各指标作用下次均医疗费用的变动。结果 2010—2016年全国医院次均门诊费用环比增长了55.60%,次均住院费用环比增长了87.83%。期间年门诊诊疗人次和出院人数,以及千人口执业医师(助理)数对次均医疗费用的作用程度较为明显,人口数这一因素对次均医疗费用的影响最小。结论加强基层医疗机构服务能力建设,提高卫生服务供给水平;进一步提高卫生人力资源配置在人口学上的公平性,降低次均医疗费用,从而减轻患者的就医负担。

Objective Through the study of the main influencing factors of the average medical expenses in China,to provide the basis of controlling the growth of the average medical expenses.Methods Collecting the expense data of hospital in China from2010to2016,and using Chain Substitution Method to analyze the impact of various factors on the average medical costs.Results The average cost of outpatient clinics in China increased by55.60%on ring growth,and the average cost of hospitalization increased by87.83%on ring growth.The most influential factors for the growth of average medical expenses during the period were the increase in annual outpatient visits and annual discharge,and the minimal impact of average medical costs was population.Conclusion The service capacity of basic medical institutions should be strengthened,and the level of provision of health services be improved.Then the demo-graphical fairness in medical technician should be improved,and the average medical costs be reduced to decrease the patients’medical burden.
