经皮椎间孔镜治疗腰椎间盘突出症临床疗效研究

Effect of percutaneous endoscopic lumbar discectomy in 80 patients with lumbar disc herniation

目的探讨经皮椎间孔镜治疗腰椎间盘突出症的临床效果。方法选取自2015年1月至2018年1月沈阳市骨科医院收治的80例腰椎间盘突出症患者为研究对象。采用随机数字表法将患者分为观察组与常规组,每组各40例。常规组患者采取传统腰椎间盘突出症开窗髓核摘除术治疗,观察组采取经皮椎间孔镜治疗。比较两组患者的手术时间、住院时间、并发症发生率,术前、术后的疼痛视觉模拟评分量表(VAS)评分及腰椎功能障碍指数(ODI),采取改良Macnab标准评价治疗效果。结果观察组手术时间、住院时间、并发症发生率均显著低于常规组(P <0. 05);术后观察组的VAS评分及ODI评分均显著优于常规组(P <0. 05)。观察组临床疗效优良率为95. 0%(38/40),显著高于常规组的80. 0%(32/40),两组比较,差异有统计学意义(P <0. 05)。结论采取经皮椎间孔镜治疗腰椎间盘突出症,可减少手术时间,降低并发症发生率,缩短术后康复时间,减轻疼痛,改善功能障碍,临床疗效显著。

Objective To investigate the clinical effect of percutaneous endoscopic lumbar discectomy(PELD)in patients with lumbar disc herniation.Methods A retrospective study was performed on 80 cases of patient with lumbar disc herniation who were admitted from January 2015 to January 2018.Patients were randomly divided into the observation group and the conventional group,with 40 cases in each group.Patients in the conventional group were treated with traditional resection of lumbar intervertebral disc herniation and those in the observation group were treated with PELD.The operation time,hospitalization time,complication incidence,preoperative and postoperative visual analogue scale(VAS)score and lumbar dysfunction index(ODI)of patients in the two groups were compared,and improved Macnab standard was adopted to evaluate the treatment effect.Results The operation time,the time of hospitalization and the incidence of complications in the observation group were significantly lower than that of the conventional group(P<0.05).The VAS and ODI scores of the observation group were significantly better than those of the conventional group(P<0.05).The excellent and good rate of the observation group was 95.0%(38/40),significantly higher than 80.0%(32/40)of the conventional group(P<0.05).Conclusion The treatment of lumbar intervertebral disc herniation by percutaneous endoscopic lumbar discectomy can reduce the operation time,reduce the incidence of complications,shorten the postoperative rehabilitation time,reduce pain,improve the efficacy and improve the dysfunction.
