垂体腺瘤的手术治疗分析

Analysis of curative effect of surgery on patients with pituitary adenomas(report of 116 cases)

目的探讨垂体腺瘤的手术治疗方法及效果。方法回顾性分析2011年2月至2017年10月手术治疗的116例垂体腺瘤的临床资料。经单侧鼻蝶窦入路85例,标准翼点入路24例,右眉弓锁孔入路4例,冠状瓣纵裂入路3例。结果肿瘤全切除92例,次全切除20例,大部切除3例,部分切除1例。术后发生一过性尿崩12例,脑脊液漏6例,明显垂体功能低下5例。结论经单侧鼻蝶窦入路是目前首选的手术方法,但对于侵袭性生长的垂体腺瘤需根据肿瘤影像特点选择适当的手术入路。

Objective To evaluate the curative effect of different surgical forms on the pituitary adenomas,and their indications,merits and demerits.Methods The clinical data of116patients with pituitary adenomas,who underwent surgery from February,2011to October,2017,were analyzed retrospectively.Of116patients with pituitary adenomas,85were treated by transnasal transphenoidal microsurgery,24by microsurgery via pterional approach,4microsurgery via right supercillary arche key hole approach and3by microsurgery via longtudinal approach.Results The pituitary adenomas were totally resected in92patients,subtotally in20,in great part in3and partly in1.Conclusions The surgical outcomes are good in these116patients.The pituitary adenomas confined to the sella and parasellar regions should be removed by microsurgery through transnasal transsphenoidal approach.But the invasive pituitary tumors should be by individualized microsurgery selected according to their characteristics on the images.
