多囊卵巢综合征超声表现及内分泌指标在不同体型妇女中的相关性分析

Correlation between ultrasonographic manifestations and endocrine parameters in women with polycystic ovary syndrome of different body types

目的观察不同体型妇女多囊卵巢综合征的超声表现及内分泌指标。方法回顾性分析122例多囊卵巢综合征患者的临床资料,依据体重指数(BMI)将入组患者分为肥胖组(BMI≥24 kg/m^2) 28例及正常组(BMI<24 kg/m^2)患者94例;根据超声检查结果比较两组患者的卵巢直径、卵巢体积、卵泡个数、卵巢间质动脉阻力指数(RI)及子宫动脉阻力指数,根据血液化验结果比较两组患者的空腹血糖值(FPG)、空腹胰岛素值(FINS)、促黄体素(LH)、促卵泡激素(FSH)、促黄体素同促卵泡激素的比值(LH/LSH)、血清雌二醇(E2)、催乳素(PRL)、黄体酮(P)、游离睾酮(FT)、胰岛抵抗指数(HOMA-IR)等血清学内分泌浓度指数。结果肥胖组与正常组在平均卵巢直径、平均卵巢体积、平均卵泡个数、FINS、HOMA-IR、FSH比较均具有统计学意义(P<0.05);两组患者卵泡个数与FT及LH/FSH浓度呈显著正相关(P<0.05)。结论不同体型妇女多囊卵巢综合征的超声表现及内分泌指标均有一定程度的差异,且存在一定的相关性。

Objective To determine the possible correlation between the signs of polycystic ovary syndrome(PCOS)as manifested by ultrasound.Methods Firstly,the clinical data of 122 patients of polycystic ovary syndrome were subjected to retrospective analysis.The subjects were then divided into two groups as per BMI,namely,overweight group(BMI≥24 kg/m^2;n=28)and normal weight group(BMI<24 kg/m^2;n=94).The two groups were then compared in ovarian diameter,ovarian volume,follicle number,ovarian stromal artery resistance index(RI)and uterine artery resistance index,as parameters resulting from the ultra-sonography conducted.The two groups were also compared in terms of fasting blood glucose values,fasting insulin values(FINS),luteinizing hormone,follicle stimulating hormone LH,FSH,the ratio of luteinizing hormone and follicle stimulating hormone(LH/LSH),serum estradiol(E2),prolactin(PRL),progesterone(P),free testosterone(FT),insulin resistance index(HOMA-IR)and serum endocrine concentration index,as data resulting from blood tests.Results Statistically significant differences were found between the two groups in mean ovarian diameter,mean ovarian volume,average follicle number,FINS,HOMA-IR and FSH(P<0.05).Positive correlation between the number of follicles in the two groups and the concentration of FT and LH/FSH was identified(P<0.05).Conclusions Differences can be observed between PCOS women of different body types in endocrine parameters as well as symptoms as displayed by ultrasound.Positive correlation between the two can be determined.
