纤维支气管镜吸痰治疗心胸外科术后机械通气并发肺部感染临床研究

Clinical Study of Sputum Aspiration After Fiberoptic Bronchoscopy in Patients with Mechanical Ventilation Complicated with Pulmonary Infection After Cardiothoracic Surgery

目的研究心胸外科术后机械通气并发肺部感染患者采用纤维支气管镜吸痰的效果。方法将84例2016年3月—2018年1月在焦作市第二人民医院行心胸外科手术治疗且手术机械通气并发肺部感染的患者分为参照组和研究组,给予患者常规抗感染治疗,在此基础上给予参照组密闭式导管吸痰治疗,而研究组行纤维支气管镜吸痰治疗。对比两组的动脉血氧分压和氧合指数的变化情况,比较两组的症状消失用时,并将两组的临床治疗效果进行对比。结果治疗前,研究组和参照组的氧合指数均较低,经过治疗,两组的氧合指数均有所升高,且研究组的升高程度大于参照组。参照组的各种症状的缓解用时少于参照组。参照组的治疗总有效率(76. 19%)低于研究组(92. 86%)。结论在心胸外科术后机械通气并发肺部感染患者的临床治疗中,纤维支气管镜吸痰可加快缓解患者的症状,有利于改善患者的呼吸功能,临床疗效颇为显著,建议推广。

Objective To study the effect of bronchofiberscope aspiration in patients with pulmonary infection after mechanical ventilation after cardiothoracic surgery.Methods84patients who underwent cardiothoracic surgery from March,2016to January,2018were divided into two groups:the control group and the study group.On this basis,the control group was treated with closed catheter aspiration,while the study group was treated with fiberoptic bronchoscopy.The changes of arterial partial pressure of oxygen and oxygenation index were compared between the two groups,and the clinical effects of the two groups were compared when the symptoms disappeared.Results Before treatment,the oxygenation index of both the study group and the control group was low,after treatment,the oxygenation index of the two groups increased,and the degree of the increase of the study group was higher than that of the control group.The duration of remission of various symptoms in study group was less than that in the control group.The total effective rate of the control group(76.19%)was lower than that of the study group(92.86%).Conclusion In the clinical treatment of mechanical ventilation complicated with pulmonary infection after cardiothoracic surgery,bronchofiberoscopic sputum aspiration can accelerate the relief of patients symptoms and improve their respiratory work.The clinical curative effect is quite remarkable,so it is worth popularizing.
