自噬在膀胱癌治疗中的应用价值

摘要 目的探讨自噬在膀胱癌治疗中的应用价值。方法选取我院2014年3月至2016年2月收治的36例膀胱癌晚期患者作为研究对象,患者均接受化疗,化疗方案均为吉西他滨+紫杉醇。根据化疗效果的不同将患者分为良好组(24例)和一般组(12例),在膀胱镜下采集两组患者的癌组织进行免疫组化分析,检测自噬基因Beclin 1的表达情况,并根据自噬基因Beclin 1检测结果对36例患者进行重新分组,分为阳性组(21例)和阴性组(15例)。两组患者均随访2年,比较两组患者的预后情况。结果良好组的癌组织自噬基因Beclin 1阳性率为70.83%,显著高于一般组的33.33%,差异具有统计学意义(P<0.05)。36例患者化疗后均获有效随访,阳性组患者随访2年时的生存率为85.71%,显著高于阴性组的46.67%,差异具有统计学意义(P<0.05)。结论自噬基因Beclin 1可能具有抑癌作用,其阳性表达可能为膀胱癌患者获得良好疗效的促进因素。 Objective To explore the application value of autophagy in the treatment of bladder cancer.Methods A total of36patients with advanced bladder cancer treated in our hospital from March2014to February2016were selected as the study objects,which all received chemotherapy,and the chemotherapy regimens were gemcitabine and paclitaxel.The patients were divided into good group(n=24)and general group(n=12)according to different chemotherapy effects.The cancer tissues of the two groups were collected under cystoscopy for immunohistochemical analysis,the expression of autophagy gene Beclin1was detected.According to Beclin1test results of autophagy gene,36patients were re-grouped into positive group(n=21)and negative group(n=15).The two groups were followed up for2years and the prognosis of the two groups was compared.Results The positive rate of autophagy gene Beclin1in the good group was70.83%,which was significantly higher than33.33%in the general group,the difference was statistically significant(P<0.05).A total of36patients were followed up effectively after chemotherapy,the survival rate of the positive group was85.71%at2-year follow-up,which was significantly higher than46.67%of the negative group,and the difference was statistically significant(P<0.05).Conclusion The autophagy gene Beclin1may have anti-cancer effect,and its positive expression may be a contributing factor to obtain a good therapeutic effect in patients with bladder cancer.
