1例BCR-ABL1融合基因阴性、Ph染色体阳性的急性淋巴细胞白血病

摘要 BCR-ABL1融合基因是由Ph染色体即t(9;22)(q34;q11)异位后产生的,其转录后形成的BCR-ABL1融合蛋白具有酪氨酸激酶活性,可干扰细胞正常调控,抑制细胞凋亡,从而可引起白血病发生[1]。Ph是急性淋巴细胞白血病(ALL)患者中最常见的染色体异常,以BCR-ABL1融合基因阳性为特征[2]。Ph染色体阳性的ALL是成年人ALL中最常见的类型,约占成年人ALL的20%~30%左右,并且随着年龄的增长,其发病率有升高的趋势,在50岁以上成年人ALL中,Ph+ALL约占50%以上[3-5]。从理论上来说,Ph染色体阳性,分子生物学基础为BCR-ABL1融合基因阳性。即使存在隐匿染色体易位即Ph染色体阴性,也会存在分子生物学的改变,致使BCR-ABL1融合基因阳性。但实际的检测中会出现Ph染色体阳性而BCR-ABL1 mRNA qPCR阴性的结果,现就一例出现此结果的特殊病例进行报道,并对原因进行分析。
