摘要
目的探讨高浓度磁共振对比剂钆布醇在腹部MRA中的应用价值。方法采用1. 0M钆布醇和传统0. 5M钆喷酸葡胺对20名志愿者分两次行上腹部动脉期、门脉期的快速小角度反转梯度回波(flash) 3D MRA及平衡期的三维容积内插值法屏气检查(vibe),获取:(1)腹主动脉、腹腔干、肠系膜上动脉和肾动脉主干、门静脉汇合处、门静脉左右支、肠系膜上静脉及肝静脉汇合处的信噪比(SNR);(2)两种对比剂在腹主动脉、肝总动脉、肝固有动脉、胃十二指肠动脉、胃左动脉、肾脾动脉及分支的显示差异。结果钆布醇所获腹主动脉、腹腔干动脉、肠系膜上动脉、肾动脉主干、门静脉、门静脉左支、门静脉右支、肠系膜上静脉的SNR高于钆喷酸葡胺,差异有统计学意义(P<0. 05)。肝静脉汇合处,两种对比剂所得血管的SNR无明显差异(P=0. 078)。钆布醇在胃十二指肠动脉、脾动脉二级分支的显示优于钆喷酸葡胺,但在其余动脉血管及分支的显示上,两种对比剂差异无统计学意义(P>0. 05)。结论 1. 0M钆布醇对腹部血管,尤其是小血管的显示具有优势。
Objective To evaluate the value of gadolinium-butanol,a high concentration magnetic resonance contrast agent,in abdominal MRA.Methods 1. 0 M gadolinium-butanol and 0. 5 M gadolinium glucomine were used to examine 20 volunteers for rapid small-angle inversion gradient echo( flash) 3 D MRA in epigastric arterial phase and portal phase,and three-dimensional volumetric interpolation method in equilibrium phase( vibe) respectively.And we obtained the following data:(1)Signal Noise Ratio( SNR) of of abdominal aorta,celiac trunk,superior mesenteric artery and renal artery,portal vein confluence,left and right portal vein,confluence of the superior mesenteric vein and the hepatic vein;(2)The differences between the two contrasts in abdominal aorta,common hepatic artery,hepatic proper artery,gastroduodenal artery,left gastric artery,renal and splenic artery and branches were observed.Results The SNR of abdominal aorta,celiac trunk,superior mesenteric artery,main renal artery,left portal vein,right portal vein and superior mesenteric vein obtained by gadolinium-butanol were higher than those of meglumine gadolinate( P<0. 05).There was no significant difference in SNR between the two contrast agents at the confluence of hepatic veins( P =0. 078). Gadolinium-butanol in the secondary branches of gastroduodenal artery and splenic artery were better than that of meglumine gadolinium poumate.However,there was no significant difference between the two kinds of contrast agents in the display of other arterial vessels and branches( P>0. 05).Conclusion 1. 0 M gadolinium-butanol has an advantage in displaying abdominal vessels,especially in small vessels.
