控制良好的2型糖尿病及合并高血压患者动态动脉硬化指数与左室长轴收缩期峰值应变的相关性研究

Correlation between the Dynamic Arteriosclerosis Index of Well-controlled Type 2 Diabetes Patients with Hypertension and the Left Ventricular Long-Axis Systolic Peak Strain

目的探讨控制良好的2型糖尿病及合并高血压患者动态动脉硬化指数(AASI)与左室长轴收缩期峰值应变(GLPS)改变的关系。方法选取2017年10月至2018年4月于我院住院,已确诊的2型糖尿病患者102例,根据是否合并高血压分组为糖尿病(T2DM)组54例,糖尿病合并高血压(T2DM+HBP)组48例,另选取同时间段内健康体检者50例,作为对照组(NC)。通过心脏彩超检测心脏常规指标及左室整体应变。通过检测24小时动态血压,计算AASI,分析AASI与GLPS的相关性。结果 GLPS_Avg、GLPS_LAX、GLPS_A4C绝对值从NC组到T2DM+HBP组在三组间依次降低,T2DM+HBP组GLPS_A2C绝对值低于NC组,差异均有统计学意义(均P<0. 05); AASI在三组间依次升高,三组间差异均有统计学意义(均P<0. 05); AASI与GLPS_Avg、GLPS_LAX、GLPS_A4C、GLPS_A2C均有中等程度的正相关性(r=0. 652,r=0. 622,r=0. 631,r=0. 534,P<0. 01)。结论动态动脉硬化指数能反映2型糖尿病患者整体动脉弹性功能,与左室长轴整体峰值应变正相关,对早期监测预防糖尿病亚临床心脏、血管损害具有重要意义。

Objective To investigate the correlation between the dynamic arteriosclerosis index(AASI)and the left ventricular long-axis systolic peak strain(GLPS)in patients with well-controlled type2diabetes mellitus and hypertension.Methods A total of102patients with type2diabetes mellitus who were diagnosed in our hospital from October2017to April2018were enrolled.And according to whether or not hypertension was included,they were divided into two groups.There were54cases in the diabetic(T2DM)group and48cases in the diabetic hypertension(T2DM+HBP)group.50healthy cases in the same interval were selected as the control group(NC).Heart routine indicators and left ventricular global strain were detected by cardiac color Doppler ultrasound.AASI was calculated by detecting24-hour ambulatory blood pressure,and the correlation between AASI and GLPS was analyzed.Results The absolute values of GLPS_Avg,GLPS_LAX and GLPS_A4C decreased from NC group to T2DM+HBP group in three groups.The absolute value of GLPS_A2C in T2DM+HBP group was lower than that in NC group(P<0.05).AASI increased in the three groups,and the differences among the three groups were statistically significant(P<0.05).AASI had a moderate positive correlation with GLPS_Avg,GLPS_LAX,GLPS_A4C,and GLPS_A2C(r=0.652,r=0.622,r=0.631,r=0.534,P<0.01).Conclusion The dynamic arteriosclerosis index can reflect the overall arterial elasticity of patients with type2diabetes,and is positively correlated with the overall peak strain of the left ventricular long axis.It is of great significance for early monitoring and prevention of subclinical heart and vascular damage in diabetes.
