以家庭为中心的临床护理方案对小儿发育性髋关节脱位术后肢体功能康复及患儿家属满意度的影响

Effects of family-centered clinical nursing programs on limb function rehabilitation and family satisfaction after developmental hip dislocation in children

目的探讨以家庭为中心的临床护理方案对小儿发育性髋关节脱位术后肢体功能康复及患儿家属满意度的影响。方法选取2016年4月～2018年4月中国医科大学附属盛京医院收治的行手术治疗的88例小儿发育性髋关节脱位患者,采用随机数字表法将其分为对照组和观察组,每组各44例。对照组在术后给予常规护理,观察组在对照组的基础上加用以家庭为中心的临床护理方案。比较两组患儿肢体功能康复情况、患儿家属护理满意度评分及术后并发症发生率。结果两组患儿护理后简式Fugl-Meyer运动功能量表及Barthel评分均较护理前明显升高,且观察组高于对照组,差异有统计学意义(P <0.05)。观察组患儿家属护理态度评分、护理舒适性评分、沟通能力评分及综合评分均高于对照组,差异有统计学意义(P <0.05)。观察组患儿便秘、腹胀、皮肤破损及切口感染的发生率低于对照组,差异有统计学意义(P <0.05)。结论以家庭为中心的临床护理方案优于常规护理,其应用于小儿发育性髋关节脱位术后可促进改善患儿的肢体功能,且各类并发症发生率较低,患儿家属在各个方面的满意度均有所提高。

Objective To investigate the effect of family-centered clinical nursing program on limb function rehabilitation and family satisfaction of children with developmental dislocation of the hip.Methods A total of88patients with pediatric developmental dislocation of the hips who underwent surgical treatment from April2006to April2018in Shengjing Hospital of China Medical University were divided into control group and observation group by random number table.There were44cases in each group.The control group received routine care after surgery,and the observation group added a family-centered clinical care program based on the control group.The rehabilitation of limb function,the satisfaction score of family members and the incidence of postoperative complications were compared between the two groups.Results The simplified Fugl-Meyer assessment(FMA)and Barthel scores of the two groups were significantly higher than those before the nursing,and the differences were statistically significant(P<0.05).The nursing attitude score,nursing comfort score,communication ability score and comprehensive score of the family members in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).The incidence of constipation,abdominal distension,skin damage and wound infection in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The family-centered clinical nursing program is superior to conventional nursing.It can improve the limb function of children after developmental dislocation of the hip,and the incidence of various complications is low.The family members′satisfaction in all aspects has improved.
