5种方法在梅毒筛查中的应用评价

Five methods in the evaluation of syphilis screening

目的评价梅毒螺旋体明胶凝集试验(TPPA)、梅毒螺旋体酶联免疫吸附试验(TP-ELISA)、梅毒螺旋体抗体检测胶体金法(金标法)、梅毒螺旋体快速血浆反应素试验(RPR)和梅毒螺旋体甲苯胺红不加热血清试验(TRUST)在梅毒筛查中的应用价值。方法用临床常用的TPPA、TP-ELISA、金标法、RPR和TRUST对80例阳性标本及70例阴性标本分别进行检测。结果TPPA、TP-ELISA、金标法、RPR和TRUST的灵敏度分别为100%、98.75%、90.00%、72.5%,75%;特异性分别为100%、94.28%、91.42%、82.85%、85.71%。结论TPPA不适合大规模的梅毒筛查,适用于筛查后阳性标本梅毒抗体的确证试验。TP-ELISA是目前梅毒筛查的理想方法。金标法快速方便,适用于急诊标本。RPR和TRUST检测梅毒非特异性抗体,可用于疗效观察。

Objective To evaluate the value of Treponema pallidum gel test(TPPA),syphilis enzyme-linked immunosorbent assay(TP-ELISA),Treponema pallidum antibody colloidal gold strips(colloidal gold),Treponema pallidum antibody rapid plasma reagin test(RPR)and syphilis toluidine red untreated serum test(TRUST)in the screening of syphilis.Methods Eighty positive samples and 70 negative samples were detected by TPPA,TP-ELISA,colloidal gold method,RPR and TRUST.Results The sensitivities of TPPA,TP-ELISA,gold standard,RPR and TRUST were 100%,98.75%,90.00%,72.5%and 75%,and the specificity were 100%,94.28%,91.42%,82.85%and 85.71%.Conclusion TPPA is not suitable for large-scale screening of syphilis,suitable for positive samples after screening syphilis antibody confirmatory test.TP-ELISA is the best method of screening for syphilis.Gold standard method fast and convenient,suitable for emergency specimens.RPR and TRUST non-specific detection of syphilis antibodies can be used for observation of the curative effect.
