补肾健骨汤对骨质疏松性压缩骨折术后愈合及腰椎功能的影响

Effects of Bushen Jiangu Decoction on postoperative healing and lumbar function in patients with osteoporotic compression fracture

目的探讨补肾健骨汤对骨质疏松性压缩骨折(OVCF)术后愈合及腰椎功能的影响。方法选取2016年1月～2018年1月盘锦市中心医院收治的93例OVCF患者作为研究对象,按照随机数字表法将93例患者分为对照组(n=46)和研究组(n=47)。对照组采用维D钙咀嚼片治疗,研究组在对照组治疗基础上加用补肾健骨汤治疗。两组患者连续治疗14 d。治疗后统计两组临床总有效率,分别于治疗前后评价两组患者视觉模拟量表(VAS)和日本骨科协会腰痛评价表(JOA)评分、腰椎L2～L4骨密度、股骨大转子骨密度、股骨颈骨密度及伤椎楔变角、后凸Cobb角、伤椎椎体前缘高度比,以及不良反应发生情况。结果研究组临床总有效率明显高于对照组,差异具有统计学意义(P <0.05)。治疗后,两组患者VAS评分、伤椎楔变角、后凸Cobb角小于治疗前,研究组小于对照组(P <0.05),而JOA评分、腰椎L_2～L_4骨密度、股骨大转子骨密度、股骨颈骨密度、伤椎椎体前缘高度比明显大于治疗前,且研究组均大于对照组(P <0.05)。两组治疗过程中均未发生严重不良反应。结论补肾健骨汤治疗OVCF的临床效果显著,能够明显改善疼痛和临床症状,促进术后骨折愈合,改善患者腰椎功能,且安全性较好,为临床治疗OVCF提供了新的思路。

Objective To investigate the effect of Bushen Jiangu Decoction on postoperative healing and lumbar function in patients with osteoporotic compression fracture(OVCF).Methods A total of93cases of OVCF admitted to Panjin Central Hospital from January2016to January2018were selected as study subjects,and according to the random number table method,they were divided into control group(n=46)and study group(n=47).The patients in the control group were treated with Vitamin D Calcium Chewable Tablets,while the patients in the study group were treated with Bushen Jiangu Decoction on the basis of the treatment in the control group.They were treated for14days.Total clinical effective rate of the two groups after treatment was counted.The visual analogue scale(VAS)and Japanese Orthopedic Association low back pain scale(JOA)scores,bone mineral density of lumbar spine L2-L4,trochanter of femur,femur and the wedge angle of the injured vertebra,the Cobb angle of kyphosis and ratio of anterior vertebral height of injured vertebrae were measured before and after treatment.The adverse reactions of the two groups were observed.Results The total clinical effective rate of the study group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the VAS score,the wedge angle of the injured vertebra,the Cobb angle of kyphosis of the two groups were smaller than those before treatment,and those in the study group were less than the control group(P<0.05),while the JOA score,bone mineral density of lumbar spine L2-L4,trochanter of femur,neck of femur,and ratio of anterior vertebral height of injured vertebrae were significantly higher than those before treatment,and those in the study group were better than the control group(P<0.05).No serious adverse reactions occurred in the two groups during treatment.Conclusion The clinical curative effect of Bushen Jiangu Decoction in the treatment of OVCF is significant,which can significantly improve pain and clinical symptoms,promote postoperative fracture healing and improve lumbar function.It has high security,that provides a new thinking for clinical treatment of OVCF.
