摘要
目的:探索超高b值DWI对前列腺癌的临床诊断价值。方法:对40例临床确诊为前列腺癌患者(经临床病理穿刺证实)的MRI数据进行回顾性分析,MRI数据包括横轴位T2WI,常规b值(b=1000s/mm2) DWI以及超高b值(3000s/mm2) DWI序列图像,比较这些不同序列对诊断前列腺癌的诊断率、特异性和敏感性的差异。结果:40例前列腺癌患者发现病灶共86个,其中位于外周带者56个,位于移行区者30个。T2WI对外周带和移行区的前列腺癌的诊断率分别为66. 07%(37/56)和66. 67%(20/30),常规DWI对外周带和移行区前列腺癌的诊断率分别为58. 93%(33/56)和56. 67%(17/30)。超高b值(b=3000s/mm2) DWI对外周带和移行区前列腺癌的诊断率分别为83. 93%(47/56)和90%(27/30)。超高b值(b=3000s/mm2) DWI对外周带和移行区前列腺癌的特异性分别为68. 89%和88. 42%,敏感度分别为92. 23%和88%,均明显高于常规DWI和T2WI(p <0. 05)。结论:超高b值(b=3000s/mm2)对前列腺癌具有较高的诊断率,对前列腺癌的诊断具有重要应用价值。
Objective:Aimed to explore the diagnostic value of Ultra-high b value diffusion-weighted imaging in prostate cancerMethods:MRI data,including T2WI,conventional b value of 1000s/mm2and ultra-high b value of 3000s/mm2,of 40cases of prostate cancer confirmed by biopsy were retrospectively analyzed.The diagnosis rate,sensitivity and specificity of different MRI sequence was then caculated and compared.Results:There were total 86 lesions identified as prostate cancer including 56 in peripheral zone and 30 in transition zone.The diagnosis rate of T2WI for prostate cancer in peripheral zone and transition zone were 66.07%(37/56)and 66.67%(20/30)respectively.The diagnosis rate of conventional b value DWI in peripheral zone and transition zone were58.93%(33/56)and56.67%(17/30)respectively.The diagnosis rate of ultra-high b value DWI in peripheral zone and transition zone were 83.93%(47/56) and 90%(27/30) respectively.The specificity of ultra-high b value DWI in peripheral zone and transition zone were 68.89% and 88.42% respectively,and the sensitivity of which were 92.23% and 88%respectively,all of which were significantly higher than conventional b value.Conclusion:Ultra-high b value DWI(3000s/mm2)can improve the detection rate of diagnosis for prostate cancer that it could be valuable for prostate cancer diagnosis.
