卒中后睡眠障碍对认知功能影响的研究 被引量:3

摘要 目的探讨卒中后睡眠障碍(Post-Stroke Sleep Disorders,PSSD)对卒中患者认知功能的影响。方法方便选择2016年8月—2018年2月住院并在门诊随访的391例急性卒中恢复期患者为研究对象,对所有患者收集一般临床资料,并进行PSQI、MMSE、NIHSS评定,对睡眠状况(PSQI)与认知功能障碍(MMSE)进行Pearson相关性分析及单因素、多因素回归分析。结果该研究中PSQI的分值与MMSE分值的相关性,差异有统计学意义(P=0.00<0.05),且相关系数为-0.853,说明是负相关;单因素回归分析研究结果显示,年龄、文化程度、卒中侧别、NIHSS评分、PSQI分值对MMSE影响,差异有统计学意义(P<0.05),且PSQI分值对MMSE影响B值为正数,是正相关,说明PSQI分值越高,睡眠障碍越严重,发生认知障碍的风险就越高;通过多因素回归分析结果显示PSQI分值、文化程度、NIHSS评分对MMSE的影响,差异有统计学意义(P<0.05),可见,即使在调整了年龄、文化程度、卒中侧别、NIHSS评分后,睡眠障碍对认知功能影响仍然是非常显著的。结论 PSSD患者认知功能明显损害,而这种损害的发生风险与睡眠障碍的程度密切相关,睡眠问题越严重的患者,发生认知功能损害的风险也就越高。 Objective To study the effect of dyssomnia after stroke on the cognitive function.Methods391cases of patients with acute stroke during the recovery period after the outpatient follow-up from August2016to February2018were convenient selected as the research objects,and the general clinical data of all patients were collected,and were for PSQI,MMSE,NIHSS evaluation,and the PSQI and MMSE were for Pearson related analysis and single factor and multi factor regression analysis.Results The correlation P value of PSQI and MMSE vale,with statistical significance(P<0.05),and the correlation coefficient-0.853,which showed that it was negatively correlated,and the single factor regression research results showed that the age,cultural degree,apoplexy side,NIHSS score,PSQI score could affect the MMSE,all P=0.000,<0.05,with statistical significance,and the PSQI score was positively correlated with the MMSE,B value,which showed that the higher the PSQI score,the more serious the dyssomnia,the higher the risk of cognitive disorder,and the multi-factor regression analysis results showed that the P value of PSQI score,culture degree,NIHSS to the MMSE,with statistical significance(P<0.05),and the effect of dyssomnia was very obvious after adjusting the age,cultural degree,apoplexy side and NIHSS score.Conclusion The cognitive function of PSSD patients has an obvious cognition,but the occurrence risk of the injury is closely related to the degree of dyssomnia,and the more serious the dyssomnia,the higher the occurrence risk of the cognition function impairment.
