行为管理在血液病患儿牙病操作治疗中的应用

Application of behavioral management in dental operative treatment of children with hematological diseases

目的总结近年来血液病患儿就诊于口腔科进行牙病临床操作治疗的经验,探讨行为管理在血液病患儿牙病操作治疗中是否有效。方法在口腔门诊,随机选择年龄3～7岁,需要牙科治疗的患有血液系统疾病的儿童46例,随机分为试验组和对照组各23例。试验组在临床治疗中运用语言交流、观摩学习、身体限制等行为管理方法,对照组仅进行常规操作治疗。评估分析2组患儿的就诊配合行为。结果试验组患儿牙科操作治疗配合度优于对照组,试验组合作完成治疗和强制完成治疗患儿的牙病操作治疗时间均短于对照组,试验组患儿就诊接受行为优于对照组,差异有统计学意义(P<0.05)。结论血液系统疾病患儿就医次数多,对医疗机构有恐惧感,在牙病临床治疗过程中应用行为管理方法正确诱导,可降低患儿的恐惧心理,增加其对医务人员的信任,从而配合治疗,促进其牙病治疗的顺利完成,减少口腔感染率,减轻其痛苦。

Objective To summarize the experience of clinical treatment of dental diseases in recent years in children with hematologic diseases,to investigate the effectiveness of behavior management in the treatment of dental diseases in children with hematologic diseases.Methods In oral clinic,46children aged3to7years with hematological diseases were randomly divided into two groups:the experimental group(n=23)and the control group(n=23).The experimental group used behavioral management methods such as language communication,observation and learning,and physical restriction in clinical treatment,while the control group was treated only with routine operation.The coordination behaviors of2groups of children were evaluated and analyzed.Results The cooperation of dental operation in the experimental group was better than that in the control group,the time of dental operation in the experimental group was shorter than that in the control group,and the receiving behavior of the children in the experimental group was better than that in the control group.The difference was statistically significant(P<0.05).Conclusion Children with blood system diseases have more times of going to medical treatment,have a sense of fear about the medical institutions themselves.The correct induction of behavior management in the clinical treatment of dental diseases can reduce the fear of children,increase the trust of medical staff,and promote the successful completion of dental treatment and reduce the oral infection rate,thereby alleviate the pain of children.
