大型颌骨囊性病变的开窗减压治疗

Application of fenestration for treatment of large jaw cystic lesions

目的探讨大型颌骨囊性病变的开窗减压治疗效果。方法 2013—2017年在中山市人民医院·口腔分院口腔颌面外科就诊的大型颌骨囊性病变患者18例,均采用开窗减压治疗术,观察术后3年囊腔的变化情况。结果术后随访1～3年,15例患者囊腔基本消失、2例复发、1例治疗无效(改为传统的手术)。开窗治疗的病例病变区域骨质生长恢复良好,临床症状消除。结论大型颌骨囊性病变的开窗减压治疗切实可行。对于病变累及的患牙牙髓尽量保留,待囊腔周围骨质生长回后,开窗减压治疗术后1年时根据活力测试考虑是否需要根管治疗。

Objective To explore the effect of fenestration for large jaw cystic lesions.Methods Eighteen cases of large odontogenic cyst of maxillofacial areas in our department in2013-2017years were treated with fenestration,and the changes of cysts in3years after the operation were observed.Results After1-3years of follow-up,the cystic cavity disappeared in15cases,2cases recurred and1case was not effective,which was treated by traditional operation.In the cases of fenestration,the bone growth in the lesion area recovered well,and the clinical symptoms were eliminated.Conclusions Fenestration for large jaw bone cystic lesions is feasible.The pulp of the affected teeth was preserved as far as possible.After the bone growth around the capsule,the root canal therapy was considered one year after the fenestration according to the dental pulp test.
