哌拉西林他唑巴坦钠诱导免疫性溶血反应的研究

The study of piperacillin tazobactam sodium-induced immune hemolytic reaction

目的探讨哌拉西林他唑巴坦钠药物抗体的发生频率及其导致免疫性溶血反应的情况。方法应用微柱凝胶技术对临床上使用哌拉西林他唑巴坦钠治疗的患者,采集血样进行直接抗球蛋白试验(DAT),并对DAT阳性者进行哌拉西林他唑巴坦钠药物抗体检测,对药物抗体阳性者收集实验室数据及临床资料,分析其诱导免疫溶血反应的情况。结果纳入该研究的125例被检者中,DAT阳性74例(59.2%),其中检出哌拉西林他唑巴坦钠药物抗体34例(27.2%),4例患者出现症状较轻的血管外溶血反应。结论临床上使用哌拉西林他唑巴坦钠治疗过程中,机体可产生相关药物抗体而引起药源性免疫溶血反应。

Objective To explore the prevalence of piperacillin tazobactam sodium antibodies and the characteristics of these drug-induced immune hemolytic reaction.Methods The blood samples were collected from the patients treated with piperacillin tazobactam sodium and the direct antiglobulin test(DAT)of the red blood cells was done using the micro gel column technique.Detection of the drug antibodies was performed on the patients with positive DAT.Then the laboratory and clinical data of the patients with positive drug antibodies were collected to analyze the immune hemolysis reaction induced by the drug antibodies of piperacillin tazobactam sodium.Results In the125patients who were included in this study,DAT was positive in74cases(59.2%)among whom34cases(27.2%)had piperacillin-tazobactam sodium drug antibodies.Of the patients with drug antibodies,4cases occurred extravascular hemolysis with slight symptoms.Conclusion The patients treated with piperacillin tazobactam sodium may produce the related drug antibodies causing drug-induced immune hemolytic reaction.
