尿毒症腹膜透析患者血尿酸与脂代谢的关联性研究

Correlation between serum uric acid and lipid metabolism in uremia patients undergoing peritoneal dialysis

目的研究尿毒症腹膜透析患者高尿酸血症的发生率及其与血脂代谢的关系。方法将41例尿毒症腹膜透析患者按其尿酸水平分为高尿酸血症组20例和血尿酸正常组21例。检测两组的血脂、血清脂蛋白、白蛋白、葡萄糖、尿素、肌酐水平,分析尿毒症腹膜透析患者高尿酸血症的患病率,比较两组一般情况及血脂水平,并分析影响尿酸水平的危险因素。结果尿毒症腹膜透析患者高尿酸的发病率为48.78%。高尿酸血症组患者甘油三酯(TG)水平明显高于血尿酸正常组(P<0.05),两组患者血清总胆固醇(TC)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)、脂蛋白a(Lipo-a)水平比较差异无统计学意义(均P>0.05)。Logistic回归分析显示尿毒症腹膜透析患者高TG水平是高尿酸血症的危险因素(OR=6.20,P=0.02)。结论尿毒症腹膜透析患者高尿酸血症的发生率较高,高尿酸血症患者具有较高的TG水平,同时高TG是高尿酸血症的危险因素,应高度重视尿毒症腹膜透析患者血尿酸水平及其与血脂代谢的关系。

Objective To investigate the prevalence rate of hyperuricemia and the correlation between serum uric acid and lipid metabolism in uremia patients undergoing peritoneal dialysis(PD).Methods Forty-one PD patients were selected and divided into hyperuricemia group(n=20)and normal uric acid group(n=21).The levels of blood lipid,serum lipoprotein,albumin,glucose,urea as well as creatinine were evaluated.The prevalence of hyperuricemia in the PD patients was investigated and the general information and blood lipid level were compared between the two groups.Meanwhile,the risk factors related to the level of serum uric acid were analyzed.Results The incidence of hyperuricemia was48.78%in the PD patients.The levels of TG in the hyperuricemia group were significantly higher than those in the normal uric acid group(P<0.05).There were no significant differences in the serum levels of TC,HDL,LDL and Lipo-a between the two groups(P>0.05).Logistic analysis showed that the level of TG was the risk factor of hyperuricemia in uremia patients undergoing peritoneal dialysis(OR=6.20,P=0.02).Conclusion The incidence of hyperuricemia is high in the PD patients.The levels of TG in the PD patients with hyperuricemia are significantly higher than those with normal uric acid level.The level of serum uric acid is associated with triglyceride level.More attention should be paid to the level o f serum uric acid in the PD patients.
