外科Ⅲ类切口手术切口感染危险因素研究

Risk Factors of Incision Infection in Patients Undergoing Type Ⅲ Incision Surgery

目的探讨外科Ⅲ类切口手术切口感染的危险因素,为降低术后切口感染的发生提供参考。方法回顾性分析2015年至2016年玉林市第一人民医院外科Ⅲ类切口手术2 729例的临床资料,以术后是否发生切口感染为因变量,以性别、年龄等相关因素为自变量,采用非条件Logistic回归模型分析。结果 2 729例外科Ⅲ类切口手术患者中,发生术后感染184例,感染率为6. 74%; Logistic单因素分析显示,年龄和住院时间等8个相关因素与外科Ⅲ类手术切口感染有关(P <0. 05); Logistic多因素回归分析显示,年龄、手术部位、急诊手术、输血、住院时间、合并基础疾病为切口感染的独立危险因素(P <0. 01)。结论针对不同手术部位采取差别护理,避免急诊手术,严格控制输血,缩短住院时间和术前改善患者基础疾病状况,可防控外科Ⅲ类切口手术术后切口感染的发生。

Objective To investigate the risk factors of incision infection in patients undergoing typeⅢincision surgery,in order to provide reference for reducing the incision infection after surgery.Methods The clinical data of2729patients undergoing typeⅢincision surgery in The First People's Hospital of Yulin City from2015to2016were retrospective analyzed.The postoperative incision infection was taken as the dependent variable,and the relevant factors such as sex and age were taken as independent variables,the analysis was carried out by using unconditional Logistic regression model.Results Among2729patients undergoing typeⅢincision surgery,184cases had postoperative infection,and the infection rate was6.74%.Logistic single factor analysis showed that8related factors including age and length of hospitalization and so on were associated with surgical incision infection of typeⅢincision surgery(P<0.05).Logistic multi-factors regression analysis showed that age,surgical site,emergency surgery,blood transfusion,the length of hospitalization and combined with underlying diseases were independent risk factors for incision infection(P<0.01).Conclusion Differential nursing for different surgical sites,avoiding emergency surgery,strictly controlling blood transfusion,shortening the length of hospitalization and improving patients'basic disease condition before surgery can prevent and control incision infection of patients after typeⅢincision surgery.
