呼吸康复治疗老年稳定期支气管扩张症的临床观察

Clinical Observation of Pulmonary Rehabilitation on the Elderly with Stable Bronchiectasis

目的观察呼吸康复治疗对老年稳定期支气管扩张症患者的肺功能、圣乔治呼吸问卷评分和BODE指数的影响,以寻求有效的康复方法。方法选择2016年5月-2017年5月我院呼吸科住院治疗后出院的支气管扩张症稳定期患者70例,按照随机数字表的方法分成呼吸康复组(观察组)和对照组,每组35例。2组患者出院后均给予常规的健康指导,观察组在此基础上加用呼吸康复训练,总疗程为6个月。比较2组患者呼吸康复前与康复后3个月、6个月的肺功能、圣乔治呼吸问卷评分和BODE指数的变化,以及2组患者之间同一时间点同一指标的差异。结果与对照组和呼吸康复前比较,观察组呼吸康复3个月、6个月后肺功能指标FEV1、FEV1/FVC、FEF25%-75%、DLCO均有明显改善(P<0.05);圣乔治呼吸问卷评分和BODE指数均有明显下降(P<0.05)。结论呼吸康复治疗能提高老年稳定期支气管扩张症患者运动耐力,改善肺功能及患者生活质量。

Objective To observe the effect of pulmonary rehabilitation on pulmonary function test,the scores of SGRQ(modified St George's Respiratory Questionnaire)and BODE index(body mass index,airflow obstruction,dyspnea and exercise capacity index)on the elderlywith stable bronchiectasis so as to seek outmore effective rehabilitationmethods..Methods70discharged cases with stable bronchiectasis after treatment in department of respiratory medicine in the hospital during the period from May,2016 to May,2017were randomly divided into2groups:observation group and control group,35cases in each;routine health guidance was provided to all the cases in the 2 groups after discharge while pulmonary rehabilitation was added to cases in observation group;the total courses for both groups lasted for6months;the pulmonary function,the scores of SGRQ and BODE index of all the cases before rehabilitation,3and6months after rehabilitation were comparatively studied and the difference of the same index at the same time point of the cases between the 2 groups was analyzed.Results FEV1,FEV1/FVC,FEF25%-75%andDLCO of the cases in observation group3and6months after pulmonary rehabilitationweremuch more obviously improved and the scores of SGRO and BODE index were dropped more significantly than those of the cases in control group(P<0.05).Conclusions Pulmonary rehabilitation can promote the exercise performance,improve the pulmonary function and raise the quality of life of the elderly with stable bronchiectasis.
