正常成人脊柱-骨盆矢状位影像学参数的关系

Correlation between normal adult spine-pelvic sagittal imaging parameters

目的探讨正常成人脊柱-骨盆矢状位影像学参数的关系。方法选择正常健康志愿者278例,均行负重位全脊柱正侧位X线检查,通过医院影像归档和通讯系统测量骨盆矢状位、脊柱解剖学矢状位、脊柱形态学矢状位相关参数,并分析其相关性。结果骨盆矢状位相关参数:骨盆入射角为45. 1°±9. 6°,骨盆倾斜角为10. 3°±6. 7°,骶骨倾斜角为34. 8°±7. 8°;脊柱解剖学矢状位相关参数:腰椎前凸角为48. 3°±11. 1°,胸椎后凸角为24. 2°±9. 8°,颈椎前凸角为8. 6°±8. 7°,脊柱矢状位平衡为(-18. 9±29. 5) mm;脊柱形态学矢状位相关参数:腰弯前凸角为49. 0°±11. 1°,上腰弯前凸角为14. 6°±5. 9°,下腰弯前凸角为34. 5°±7. 9°,胸弯后凸角为32. 1°±9. 7°,颈弯前凸角为8. 5°±8. 7°,腰弯倾斜角为-5. 5°±4. 3°。骨盆入射角与腰椎前凸角、腰弯前凸角、下腰弯前凸角、腰弯倾斜角均具有明显相关性(P均<0. 05);骶骨倾斜角与胸椎后凸角、腰弯前凸角、上腰弯前凸角、下腰弯前凸角、胸弯后凸角、颈弯前凸角、腰弯倾斜角均具有明显相关性(P均<0. 05);骨盆入射角、骨盆倾斜角、骶骨倾斜角三者之间亦具有明显相关性(P均<0. 05);其余参数之间均无相关性(P均> 0. 05)。结论正常成人脊柱-骨盆矢状位影像学参数之间存在明显相关性。

Objective To investigate the relationship between sagittal imaging parameters of normal adult spine-pelvis.Methods A total of278normal healthy volunteers were selected.All patients underwent X-ray examination of the whole spine with weight-bearing position.The related parameters of sagittal view of pelvis,sagittal anatomy of the spine and sagittal view of spine morphology were measured by the hospital image archiving and communication system,and the relationship between them was analyzed.Results Pelvic sagittal parameters:the pelvic angle of incidence was 45.1°±9.6°,pelvic angle of inclination was 10.3°±6.7°,and sacral angle of inclination was 34.8°±7.8°;spine anatomy sagittal related parameters:the angle of lumbar lordosis was 48.3°±11.1°,the thoracic kyphosis angle was 24.2°±9.8°,the cervical lordosis angle was8.6°±8.7°,and the spinal sagittal balance was(-18.9±29.5)mm;the sagittal view of spine morphology-related parameters:the anterior angle of the lumbar curve was 49.0°±11.1°,the anterior angle of the upper lumbar curve was 14.6°±5.9°,the anterior angle of the lower lumbar curve was 34.5°±7.9°,and the angle of upper thoracic kyphosis was 32.1°±9.7°,the anterior angle of the neck bend was 8.5°±8.7°,and the anterior angle of the waist bend was -5.5°±4.3°.There were significant correlations between the pelvic incident angle and the lumbar lordosis angle,anterior angle of the lumbar curve,anterior angle of the lower lumbar curve and anterior angle of the waist bend,and between the sacral angle of inclination and the thoracic kyphosis angle,anterior angle of the lumbar curve,anterior angle of the upper lumbar curve,anterior angle of the lower lumbar curve,angle of upper thoracic kyphosis,anterior angle of the neck bend and the anterior angle of the waist bend,and between pelvic angle of incidence,pelvic angle of inclination and sacral angle of inclination(all P<0.05).There was no correlation between and among the remaining parameters(P>0.05).Conclusion There is a significant correlation between the normal adult spine-pelvic sagittal imaging parameters.
