Related factors of sarcopenia in elderly male patients with type 2 diabetes

摘要 目的探讨老年男性2型糖尿病患者肌少症的相关因素。方法纳入首都医科大学宣武医院内分泌科住院的≥60岁的男性2型糖尿病患者共174例,根据Baumgartner诊断标准分为肌少症组(54例)和非肌少症组(120例)。测定糖化血红蛋白(HBA1c),C反应蛋白(CRP),白细胞介素6(IL-6)水平,握力,四肢骨骼肌肌肉质量(ASM)。计算体质量指数(BMI),四肢骨骼肌质量指数(ASMI)。分析四肢骨骼肌质量指数与BMI、握力及炎症因子的相关性。结果肌少症组的BMI、ASM、ASMI、握力、尿酸、前白蛋白、血色素显著低于非肌少症组(P <0. 05)。肌少症组的CRP及IL-6水平显著高于非肌少症组(P <0. 05)。多元线性逐步回归分析显示BMI、握力为四肢骨骼肌质量指数的独立影响因素。结论握力为老年男性2型糖尿病患者肌少症独立影响因素。 Objective To explore the related factors of sarcopenia in elderly male patients with type 2 diabetes.Methods A total of174elderly male patients with type 2 diabetes aged more than60years were included in the study.According to the Baumgartner diagnostic criteria for sarcopenia,the patients were divided into sarcopenia group(n=54)and non-sarcopenia group(n=120).Levels of glycosylated hemoglobin(HBA1c),serum CRP and IL-6were measured.Grip strength,appendicular skeletal muscle mass(ASM)were also assessed.Body mass index(BMI)and appendicular skeletal muscle mass index(ASMI)were calculated.The relationships between ASMI and BMI,grip strength,and inflammatory cytokines were evaluated.Results Compared with patients without sarcopenia,the levels of BMI,ASM,ASMI,grip strength,uric acid,prealbumin and hemoglobin decreased in patients with sarcopenia(P<0.05),while the serum levels of CRP and IL-6significantly increased(P<0.05).In a multiple linear stepwise regression analysis,BMI and grip strength were independent related factors for ASMI.Conclusion Grip strength is an independent risk factor for sarcopenia in elderly male patients with type2diabetes.
