超声引导胸椎旁神经阻滞在食管癌患者电视胸腔镜食管切除术中的应用

Application of ultrasound-guided thoracic paravertebral block in esophagectomy under video-assisted thoracoscopy in patients with esophageal carcinoma

目的观察超声引导胸椎旁神经阻滞在食管癌患者电视胸腔镜食管切除术中的应用疗效。方法随机选取我院2017年1月～12月收治的72例食管癌患者为本次研究对象。按照麻醉方式的不同,将所有患者分为对照组与实验组,对照组36例患者行全身麻醉,实验组36例患者实施超声引导下胸椎旁神经阻滞。比较两组患者的麻醉效果。结果实验组患者T2、T3、T4同一时间段血压明显优于对照组,P <0.05。实验组患者T1、T2、T3、T4同一时间段心率、SpO_2与对照组相比,P> 0.05。实验组患者术后T5、T6、T7、T8同一时间Ramsay镇静得分与对照组相比,P> 0.05。实验组患者术后T5、T6、T7、T8同一时间段疼痛VAS得分明显低于对照组,P <0.05。结论食管癌电视胸腔镜食管切除术患者术中超声引导胸椎旁神经阻滞效果明显优于全身麻醉效果。

Objective To observe the application effect of ultrasound-guided thoracic paravertebral block in esophagectomy under video-assisted thoracoscopy in patients with esophageal carcinoma.Method 72 patients with esophageal carcinoma who were admitted and treated in our hospital from January to December 2017 were randomly selected as the research objects.According to different anesthesia methods,all patients were divided into control group and experimental group.36 patients in control group were given general anesthesia while 36 patients in experimental group were given ultrasound-guided thoracic paravertebral block.The anesthetic effects of the two groups were compared.Results The blood pressure at T2,T3 and T4 of the experimental group was significantly better than that of the control group,P<0.05.The heart rate and SpO2 at T1,T2,T3 and T4 in the experimental group were not significantly different from those in the control group,P>0.05.Ramsay sedation scores at T5,T6,T7 and T8 in the experimental group were not significantly different from those in the control group,P>0.05.The VAS scores at postoperative T5,T6,T7 and T8 of patients in the experimental group were significantly lower than those in the control group,P<0.05.Conclusion Ultrasound-guided thoracic paravertebral block in esophagectomy under videoassisted thoracoscopy in patients with esophageal carcinoma has better effect than general anesthesia.
