摘要
目的探讨α-地中海贫血基因型和红细胞参数之间的关系。方法随机抽样选取我院2012年11月~2017年11月收治的240例α-地中海贫血基因型患者作为本次研究对象,根据珠蛋白基因型不同,将所有患者分为静止组(n=85)、标准型组(n=110)和HbH组(n=45)三组。同时选取同期健康体检者200例,设为正常组(n=200)。检测和分析各组的红细胞参数,包括红细胞计数(RBC)、红细胞比容(HCT)、红细胞血红蛋白(HGB)、红细胞平均血红蛋白(MCH)、红细胞平均体积(MCV)、红细胞体积分布宽度(RDW-CV)。结果女性静止组的RDW-CV与正常组比较,差异无统计学意义(P> 0.05)。静止组、标准型组及HbH组的其余红细胞参数指标与正常组比较,差异具有统计学意义(P <0.05)。男性静止组、HbH组的RBC与正常组相比较,差异无统计学意义(P> 0.05)。静止组的RDW-CV与正常组比较,差异无统计学意义(P> 0.05)。静止组、标准型组及HbH组的其余红细胞参数指标与正常组比较,差异具有统计学意义(P <0.05)。结论表型红细胞参数值可以作为α-地中海贫血初次筛选的重要指标,在基层医院α-地中海贫血筛查工作中有较高的应用价值。
Objective To explore the relationship between the genotype ofα-thalassemia and erythrocyte parameters.Methods 240 patients withα-thalassemia genotype who were admitted and treated in our hospital from November 2012 to November 2017 were selected as research objects by random sampling.According to different globin genotypes,all patients were divided into stationary group(n=85),standard group(n=110)and HbH group(n=45).At the same time,200 subjects who underwent physical examination in the same period were selected and set as the normal group(n=200).Erythrocyte parameters of every group were detected and analyzed,including red blood cell count(RBC),hematocrit(HCT),RBC hemoglobin(HGB),RBC mean corpuscular hemoglobin(MCH),RBC mean corpuscular volume(MCV)and red blood cell volume distribution width(RDW-CV).Results There was no statistically significant difference in RDW-CV between the female stationary group and the normal group(P>0.05).The other erythrocyte parameters in stationary group,the standard group and the HbH group were different with that in the normal group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in RBC in male stationary group,HbH group and the normal group(P>0.05).There was no statistically significant difference in RDW-CV between stationary group and the normal group(P>0.05).The other erythrocyte parameters in stationary group,the standard group and the HbH group were different with that in the normal group,and the difference was statistically significant(P<0.05).Conclusion Phenotypic erythrocyte parameter value can be used as an important index of primary screening forα-thalassemia,which has high application value in screening forα-thalassemia in primary hospitals.
