翼状胬肉切除联合角膜缘干细胞移植术的临床分析

Clinical analysis of pterygium excision combined with limbal stem cell transplantation

目的探讨翼状胬肉切除联合角膜缘干细胞移植术的临床疗效。方法选择2015年月9月～2018年1月我院收治的翼状胬肉患者100例(100只眼),随机分为联合治疗组及对照组,每组50例,对照组采用游离结膜移植术,联合组给予翼状胬肉切除联合角膜缘干细胞移植术。比较两组手术情况,如恢复时间、手术时间、出血量、切口大小。术后半年至一年进行复查,观察两组术后复发率及并发症发生率。结果与对照组比较,联合组的恢复时间和手术时间明显缩短、切口明显缩小、出血量明显降低,差异有统计学意义(P <0.05)。术后随访6个月、1年,联合组的痊愈率明显高于对照组,复发率及并发症发生率明显低于对照组,差异有统计意义(P <0.05)。结论翼状胬肉切除联合角膜缘干细胞移植术可明显缩短恢复时间和手术时间、缩小手术切口大小、降低出血量,却术后复发率底,安全有效,适合临床应用及推广。

Objective To explore the clinical curative effect of pterygium excision combined with limbal stem cell transplantation.Methods One hundred patients(100 eyes)with pterygium who were admitted and treated in our hospital from September 2015 to January 2018 were selected and they were randomly divided into the combined treatment group and the control group,with 50 cases in each group.The control group was treated with free conjunctival transplantation while the combined group was treated with pterygium excision and limbal stem cell transplantation.The operation conditions such as recovery time,operation time,bleeding loss and incision size were compared between the two groups.The patients were given reexamination on six months to one year after operation,and the postoperative recurrence rate and complication rate of the two groups were observed.Results Compared with the control group,the recovery time and operation time of the combined group were significantly shortened,the incision size was significantly reduced and the blood loss was significantly reduced,the differences were statistically significant(P<0.05).After 6 months and 1 year follow-up,the cure rate of the combined group was significantly higher than that of the control group.The recurrence rate and complication rate were significantly lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion Pterygium excision combined with limbal stem cell transplantation can significantly shorten the recovery time and operation time,reduce the size of surgical incision and decrease the blood loss,but the recurrence rate is low.It is safe and effective,which is suitable for clinical application and promotion.
