CRP、IL-6与TNF-α在小儿下呼吸道不同病菌感染中的水平差异研究

Study on the Level Difference Among CRP,IL-6 and TNF-α in the Infection of Different Pathogens in the Lower Respiratory Tract of Children

目的研究CRP、IL-6与TNF-α在小儿下呼吸道不同病菌感染中的水平差异。方法调查1238例小儿下呼吸道感染患者,随机选择其中95例铜绿假单胞菌,61例克雷伯菌感染,44例鲍曼不动杆菌感染,共计200例患儿为研究组;同期选取健康儿童40例作为对照组。通过检测这些研究对象血清C反应蛋白、IL-6与TNF-α的水平,鉴别小儿下呼吸道感染的病源。结果鲍曼不动杆菌下呼吸道感染患者体内C反应蛋白、IL-6与TNF-α水平明显要比铜绿假单胞菌、克雷伯菌感染患者以及正常儿童升高,差异具有统计学意义(均P <0.05)。鲍曼不动杆菌下呼吸道感染组中,重度感染患者体内的CRP、TNF-α、IL-6的水平最高,其次是中度感染患者,最后是轻度感染患者,三组之间的差异显著,具有统计学意义(P <0.05)。结论小儿患者体内的C反应蛋白、IL-6与TNF-α水平可以作为鉴别小儿鲍曼不动杆菌下呼吸道感染的指标。

Objective To analyse the difference of levels among CRP,IL-6 and TNF-αin different infections of the lower respiratory tract in children.Methods 1238 cases of lower respiratory tract infections in children were investigated.Among them a total of 200 cases of children were randomly selected as research group,including 95 cases of pseudomonas aeruginosa,61 cases of klebsiella pneumoniae infection,44 cases of bauman′s acinetobacter infection.40 healthy children were selected as control group.The source of lower respiratory tract infection in children was identified by detecting the levels of serum C reactive protein,IL-6 and TNF-αlevels in these subjects.Results The levels of C reactive protein,IL-6 and TNF-αin patients with acinetobacter aeruginosa lower respiratory tract infection were significantly different from those in patients with pseudomonas aeruginosa,klebsiella pneumoniae and normal children(all P<0.05).The levels of IL-6 and TNF-αin patients with lower respiratory tract infection were statistically significant.Bauman,acinetobacter and lower respiratory tract infection group had the highest expression of CRP,TNF-αand IL-6 in the patients with severe infection,followed by moderately infected patients,and the last one was mild infection.The difference between the three groups was statistically significant(P<0.05).Conclusion The levels of C reactive protein,IL-6 and TNF-αin children can be used as indicators to identify acinetobacter baumannii lower respiratory tract infection in children.
