产妇血清IL-6、sICAM-1、CRP水平与未足月胎膜早破的相关性研究

Correlation of Serum IL-6,sICAM-1 and CRP Levels with Preterm Premature Rupture of Membranes

目的探讨产妇血清白介素-6(IL-6)、可溶性细胞间黏附分子(sICAM-1)、C反应蛋白(CRP)水平与未足月胎膜早破(PPROM)的相关性。方法抽取本院2015年9月至2017年9月住院分娩的PPROM产妇120例(观察组)及正常足月产妇100例(对照组)。观察组中合并绒毛膜羊膜炎(CA)65例,破水到分娩时间≥24h者66例。比较各组产妇血清IL-6、sICAM-1、CRP水平;Spearman相关性分析血清各指标间的关系。结果观察组产妇血清CRP、IL-6、sICAM-1水平均显著高于对照组,差异有统计学意义(P<0.05);破水到分娩时间≥24h组抗生素应用前血清CRP、IL-6、sICAM-1水平均显著高于破水到分娩时间<24h组(P<0.05);PPROM合并CA产妇血清CRP、IL-6、sICAM-1水平均显著高于未合并CA者(P<0.05);血清CRP与IL-6、sICAM-1水平显著正相关(P<0.05),血清IL-6与sICAM-1水平显著正相关(P<0.05)。结论血清CRP、IL-6、sICAM-1水平与PPROM发生有关,且其水平随破水时间延长而升高,增加CA发生风险。

Objective To investigate the correlation of serum interleukin-6(IL-6),soluble intercellular adhesion molecule(sICAM-1)and C reactive protein(CRP)levels with preterm premature rupture of membranes(PPROM).Methods 120 patients with PPROM(observation group)and 100 full-term partients(control group)who were admitted to the hospital during the period from September 2015 to September 2017 were enrolled in the study.In the observation group,there were 65 cases with histologic chorioamnionitis(CA)and 66 cases with time from membrane rupture to delivery≥24h.Serum levels of IL-6,sICAM-1 and CRP were compared between groups.The correlation between serum indexes was analyzed by Spearman correlation analysis.Results The maternal serum levels of CRP,IL-6 and sICAM-1 in the observation group were significantly higher than those in the control group(P<0.05).The serum levels of CRP,IL-6 and sICAM-1 in time form membrane rupture to delivery≥24h group before using antibiotics were significantly higher than those in time from membrane rupture to delivery<24h group(P<0.05).The maternal serum levels of CRP,IL-6 and sICAM-1 were significantly higher in partients with PPROM and CA than those without CA(P<0.05).Serum CRP was obviously positively correlated with IL-6 and sICAM-1(P<0.05),and serum IL-6 was obviously positively correlated with sICAM-1(P<0.05).Conclusion Serum levels of CRP,IL-6 and sICAM-1 are related to the occurrence of PPROM,and the levels increase with the prolongation of membrane rupture time,which increases the risk of CA.
